Objective:By studying each of peripheral diabetic nephropathy NADPH oxidase activity changes to predict the severity of diabetic nephropathyand oxidative stress,to provide a reference for clinicians to assess oxidative stress in patients and the severity of diabetic nephropathy,to better guide specific clinical application of antioxidant drugs.Methods:A case-control study of patients with diabetes admitted to hospital from October 2014 to January 2016 period in 65 cases. According to the instant urine specimens albumin /creatinine ratio value group size, normal urinary albumin into the urine of 23 patients(m AIb / Cr <30ug / mg), 22 cases of urinary micro-albumin group(m AIb / Cr30 ~ 299 ug /mg), clinical albuminuria group of 20 patients(m AIb / Cr ≧ 300 ug / mg), and choose healthy in our hospital medical center as a control group of 23 cases recorded each patient’s age, gender, disease duration, and urine microalbumin protein / urine creatinine values(m AIb / Cr), neutrophil NADPH oxidase activity, fasting blood glucose, glycated hemoglobin, systolic blood pressure, diastolic blood pressure, LDL cholesterol and triglyceride levels, comparison between groups NADPH oxidase activity levels Have significant differences, and to analyze it with microalbuminuria correlation.Results:1.Four patients in the overall level of NADPH oxidase activity mean difference was statistically significant, pairwise comparisons have been among the four groups overall mean differences were statistically significant(p <0.001), the overall average is less than the control group of diabetic normal urinary albumin group, diabetes normal urinary albumin group number is less than the overall average diabetic microalbuminuria group,diabetic microalbuminuria group is less than the overall mean of urine albumin clinical group. By pearson correlation analysis, diabetic nephropathy in diabetic patients with microalbuminuria outer peripheral neutrophil NADPH oxidase activity was positively correlated(r = 0.755, p <0.001).2.In the diabetic group, the patient’s course of disease, glycosylated hemoglobin, systolic blood pressure, LDL cholesterol, triglycerides were showed with the outer peripheral neutrophil NADPH oxidase activity positively correlated(r = 0.628, p <0.001,(r = 0.433, p<0.001)(r = 0.636, p <0.001)(r = 0.676, p <0.001)(r = 0.376, p = 0.004).Conclusion:With the aggravation of diabetic nephropathy, peripheral NADPH oxidase activity levels tended to increase, Neutrophil NADPH oxidase activity increasing with oxidative stress, may be a common basis for pathogenic mechanism for a variety of other risk factors of diabetic nephropathy, and promote the occurrence of diabetic nephropathy and development. |
