Objectives:To investigate the feasibility and safety of Percutaneous endoscopic lumbar discectomy by transiliac approach in the treatment of L5/S1 intervertebral disc herniation.Methods : retrospective analysis 8 case that suffing from L5/S1 disc herniation. and treated with PELD by transiliac approach from January 2012 to January 2015, Observe and record patients total operation time and Intraoperative fluoroscopy time,Intraoperative and postoperative complications happened or not.Evaluated and compare the preoperative and 3 days postoperative Visual Analogue Scale( VAS)scores, and Oswestry Disability Index(ODI)socres, and use the modified Macnab efficacy evaluation criteria to evaluate surgical curative effect,and CT examination to check the result of nerve root decompression. Result : 8 patients were successfully completed surgery, average operation time was 100±20min, Intraoperative fluoroscopy number was 29 times, No complications occurred inall 8 patients.The Visual Analogue Scale( VAS)scores of 3 days,3monthes, 6 monthes after the operation decreased significant when compared with preoperatively, the Oswestry Disability Index(ODI) socres also has the same change as compared with preoperative and postoperative.the modified Macnab efficacy evaluation criteria to evaluate surgical curative effect at the time of the final follow-up,Excellent was noted in 6patients, good in 1 patients, fair in 1 patient, the satisfactory rate is88.0%(7/8), the postoperative imaging comfirmed the nerve root decompressed well. Conclusion : The endoscopic lumbar discectomy by transiliac approach in the treatment of L5/S1 intervertebral disc herniation is a safe and feasible operation method, is a choice for the treatment of L5/S1 intervertebral disc herniation. |
