Background Pilocytic astrocytoma(PA) is a kind of the benign tumor that occurs in the cerebellum,the peak of which is the first twenty years of life. Relatively little is known about the behavior of PA in adults, and literature information about the characteristics of the adult PA was rare.Objective To analyse the survival(period) of Whom with PA after operation,as well as the possible factors associated with poor prognosis.Methods We retrospectively analysed the adult patients pathological diagnosed with intracranial PA, after operation, and patients were from the second affiliated hospital of Chong Qing medical university between July 2002 and July 2015. In patients with tumor characteristics, treatment methods and clinical characteristics, the results was analyzed and discussed, and the statistical data were analysed of K-M by SPSS17.0 software.Results the ultimate collection was 16 cases of patients with PA, male/female ratio is 2.2, average age was 37.6(11-65),and six patients aged less than 18 years old, 10 cases was adults. 50%(8/16) of the PA is located in the cerebral hemisphere, which was accounted for 87.5%(7/8) in adult(37.5% in temporal lobe), and 12.5%(1/8) in juvenile; followed by the cerebellum(37.5%)(6/16), and teenagers accounted for 66.7%(4/6), 33.3%(2/6) in adults; other parts including the Suprasellar and thalamus. The mean duration of clinical follow-up was 66(8-127) month. All patients, in the study, underwent surgery; six patients with tumor recurrence after operation, two of which undertook repeat surgery, one experienced radiotherapy and chemotherapy, two give up the adjuvant therapy, finally three dead, one suffered from neurological deficit. The overall survival rate was 67.9%, progression-free survival rate was 46.8%; Average overall survival was 108.57months(95%CI 90.86 to 126.28 months), the median progression-free survival was 95.46 months(95% CI, 80.15 to110.76 months), Compared with adults(> = 18) the survival rate was significantly lower than teenagers(< 18)(100% vs31.3 %, p = 0.042),And survival rate of patients older than 50 years was lower than that of patients aged less than 50(33.3% vs 83.3%, P=0.011),(100% vs 31.3, P=0.042).Conclusion The total survival rate of adults with PA is lower than teenagers, and the overall outcome is not ideal; The prognosis of patients with age greater than 50, is worse, as well as the lower survival rate; GTR helps to improve the patients’ survival rate.
