Objective: To analyze the changes of biochemical metabolites of hippocampus and and its correlation with the score of Mini-Mental State Examination(MMSE) and Montreal Cognitive Assessment(Mo CA), evaluating the roles of MRS in the early diagnosis of hippocampal damage in male gout patients, then probe the reasons for mild cognitive impairment in Gout-CI patients.Methods: A total of 35 gout patients were selected, 15 patients with mild cognitive impairment(Gout-CI) and 20 patients with cognitive normal(Gout-CN). At the same time, 15 elderly with normal cognition(matched in gender, age and education) were selected. MMSE and Mo CA were used to examine each participant and uric acid levels in all gout patients were recorded before MR examination. In addition, the N-acetylaspartate(NAA)/creatine(Cr), choline(Cho)/Cr, myo-inositol(m I)/Cr, N-acetylaspartate(NAA)/m I were obtained by ~1H-MRS scan in regions of hippocampus. Moreover, the correlations between the NAA/Cr, Cho/Cr, m I/Cr, NAA/ m I and Mo CA, MMSE scores in gout patients were analyzed respectively.Result: Compared with Gout-CN group, reduced NAA/Cr(P<0.05), increased m I/Cr(P<0.05) and reduced NAA/m I(P<0.05) were found in Gout-CI group. No significant difference was observed in the Cho/Cr between Gout-CI group and Gout-CN group(P>0.05). The values of NAA/Cr, Cho/Cr, m I/Cr, NAA/ m I between Gout-CN group and NC group have no statistically significant difference(P>0.05). The values of NAA/Cr, Cho/Cr, m I/Cr, NAA/ m I in the left and right sides of the hippocampus in gout patients have no statistically significant difference(P>0.05). In the Gout-CI group, the values of NAA/Cr, NAA/m I have remarkably positive correlation with Mo CA(P<0.05);m I/Cr has remarkably negative correlation with Mo CA(P<0.05). The average uric acid levels in the past five years in Gout-CI group are lower than Gout-CN group, and the difference was statistically significant(P<0.05).Conclusion: ~1H-MRS can early detect the changes of hippocampal damages in the patients of gout with MCI. The long-term treatment of lowering uric acid may increase the probability associated with MCI in gout patients.
