Objective: To explore the relationship among knee osteoarthritis,TCM constintution types and COL2A1 and VDR gene polymorphism by investigating KOA patients of TCM constintution types,pathogenesis risk factors and LRP5 gene polymorphism, Providing theoretical basis for improving the prevention and treatment to KOA.Methods: The study subjects245 patients with knee osteoarthritis and 54 normal controls were selected from Gansu Provincial Hospital of TCM. All the patients and normal controls were provided the informed consents. The main outcome measures including general datas,clinical informations and TCM Constitution. The database was established by Excel,statistical analysis was done by SPSS19.0.The gene polymorphism of COL2A1 and VDR was detected in some high-frequent types.Results:1. KOA femle patients were significantly more than males.The age of the patients is concentrated in 65-75 years old.2. There was no correlation between KOA onset age and sicken position(x2=6.125, P>0.05).3. In case group, the incidence of normal type in males was higher than females,and the difference was statistically significant. The top three biased constitution of KOA are Yang deficiency constitution,Yin deficiency constitution,Qi deficiency constitution;simply biased constitution of the top three are Yang deficiency constitution,Yin deficiency constitution,Qi deficiency constitution;Participate in part-folder constitution of the number of the top three are Yang deficiency constitution,Qi turbid constitution and Qi deficiency constitution.4. All the KOA patients,the severity of pain ofthem was generally from mild to moderate.The causes of the painess largely from walking stairs and squatting to standing.The characterristic of pain general are aching,swoller pain,crymodynia. The tenderness points were concentrated in the inner and outer space of the knee joint and the attachment of the medial collateral ligament.5. All KOA patients had X-ray changes.The changes mainly includes narrowing of internal and outside joint space and subchondral sclerosis.6. The gene polymorphism of COL2A1 rs2276454 locus is associated with the pathogenesis of KOA, in which the T allele may be a risk factor for the KOA patients of Yang deficiency constitution.7. There was no correlation between the VDR gene polymorphism and KOA.Conclusion:1. The risk factors of the incidence of KOA are female gender, age and TCM constitution of Yang deficiency, yin deficiency, Qi deficiency.2. There is no correlation between the age of onset of KOA and sicken position.3. X-ray changes of KOA patients are mainly narrowing of internal and outside joint space and subchondral sclerosis4. The gene polymorphism of COL2A1 rs2276454 locus is associated with the pathogenesis of KOA, in which the T allele may be a risk factor for the pathogenesis of Yang deficiency KOA.5. There was no correlation between the VDR gene polymorphism of Taq I gene and the incidence of KOA.
