The Mechanism Research Of Tongluo Decoction On Luber Intervertebral Disc Herniation

Objective The purpose of this study is to analyze the curative effect of Tongluo decoction on luber intervertebral disc herniation(LIDH), and probe into the related mechanism. So as to further express the clinical treatment method based on promoting blood circulation by removing blood stasis and invigorating vital energy with activating meridians, and provide the basis for new drug development of Traditional Chinese Medicine and the modernization of Traditional Chinese Medicine research.Methods(1)250 Kunming species mices were randomly divided according to the experiment requirement. Torsion test caused by acetic acid(chemical cause pain), hot plate method(heat cause pain) and dimethylbenzene method of ear swelling were used to evaluate the analgesia and anti-inflammatory effect of Tongluo decoction, the relevant mechanism was investigated by naloxone blocking and remove the adrenal glands experiments.(2) 50 rats were randomly divided into 6 groups: normal saline group, model control group, Tongluo decoction high, middle, low dose groups, positive control group. All rats were conduct surgery to built non-compressive lumbar nucleus pulposus prolapse animal models except normal group. Pain threshold was determined by plantar test; clotting time detected by slide method; changes of hemorheology detected by hemorheometer; the level of tumor necrosis factor-α(TNF-α), Phosphatase A2(PLA2) and prostaglandins E2(PGE2) were measured with enzyme linked immunosorbent assay;the level of nitric oxide(NO), nitric oxide synthase(NOS) were measured with colorimetric method;pathological changes of nerve root were observed by pathological detection(HE staining and immunohistochemical staining for IL-6).Results(1) In the experiment of chemical and heat cause pain, comparing with normal saline group, the sensitivity to pain decreased in positive control group and Tongluo decoction high, middle, low dose groups; and the pain threshold improved significantly(p<0.05). Meanwhile, the inflamed induced by dimethylbenzene alleviated obviously. The pain threshold reduced when pretreatment with naloxonewe. When removal the both sides of adrenal gland, the anti-inflammatory effect of Tongluo decoction disappeared.(2) In rats with LIDH, the pain threshold decreased; clotting time shorten; blood viscosity increased; the level of inflammatory cytokines such as NO,NOS,TNF-α,PLA2 and PGE2 rised; the inflammatory infiltrates were observed in nerve root tissue.Tongluo decoction could improve those symptoms, presented as increase pain threshold; prolong clotting time; lower blood viscosity; inhibiting inflammatory cytokines(NO and NOS,TNF-α,PLA2,PGE2) in serum and reduce the inflammatory cell infiltration in the nerve root tissue.Conclusion Tongluo decoction has significant analgesia and anti-inflammatory effects on luber intervertebral disc herniation(LIDH) in rats, the curative effect was associated with its ability of anti-inflammatory; analgesic action; improve blood circulation; and degrade the level of inflammatory factors(NO,NOS,TNF-α,PLA2 and PGE2). the mechanism was involved with opioid receptors and adrenal cortex.
