Objective:Retrospective analysis of the immune phenotype characteristics of 65 multiple myeloma patients and its relationship with the laboratory examination,clinical stage and classification,prognosis.Methods:Retrospective analysis of 65 newly diagnosed multiple myeloma patients in our hospital from May 2010 to August 2015.The immune phenotypic expression rate of 65 MM patients,and its relationship with the age,clinical stage and classification, laboratory and prognosis were statistically analyzed.All cases were in accordance with the criteria for diagnosis,staging and typing of Chinese multiple myeloma in 2015.Use SPSS17.0software for statistical analysis,P<0.05 were considered to be statistically significant.X±S for measurement data,Mann.Whitney inspection for clinical data,Kaplan-Meier for survival analysis,the survival of different groups were compared with bilateral Log-rank test,and Cox regression model for multivariate analysis.Results:1.Clinical data:(1)In 65 cases,40 for male and 25 for female,the ratio was 1.6:1;the median age was 62 years old.The most common type was Ig G(58.5%),the stage III was the most in MM patients;(2)The proportion of bone marrow plasma cells was40.08±24.16%,and there was no statistical difference in different ages,clinical stages and classification(P>0.05).(3)Single factor analysis showed that the hemoglobin,albumin,creatinine,beta2-micro globulin and lactate dehydrogenase wrere associated with MM prognosis,while the onset age,staging and typing,the proportion of plasma cells,serum calcium,blood sedimentation,c-reactive protein,alkaline phosphatase has nothing to do with the prognosis of MM patients.Multiple factor analysis showed that hemoglobin,beta2-microglobulin,lactate dehydrogenase had affect on the prognosis(P=0.021,0.041,0.032).2.The immune phenotype characteristics:64 patients with CD38 positive(98.5%),55 patients with CD138 positive(84.6%),36 patients with CD56 positive(55.4%),22 patients with CD117 positive(36.1%),4 patients with CD20 positive(8.0%),CD19 and CD45 were both negative.And there was no significant difference in different ages,clinical stages and classification(P>0.05).3.CD56 expression and survival analysis:(1)The median survival time of CD56 positive was 47 months(1-64 months),the median survival time was 24 months(2-41months) in patients with CD56 negative.Survival analysis showed that the survival rate of CD56 positive patients was higher than that of CD56 negative patients(P = 0.004).The result showed CD56 is a kind of prognostic indicator.(2)There were no statistical difference in hemoglobin,albumin,creatinine,beta 2-micro globulin,lactate dehydrogenase,serum calcium,blood sedimentation,c-reactive protein,alkaline phosphatase,proportion of plasma cells between CD56 positive and negative patients.4.CD117 expression and survival analysis:(1)The median survival time of CD117 positive was 15.5 months(0-64 months) of 61 cases,the median survival time was 18months(3-54 months) in patients with CD117 negative.Survival analysis showed that there was no significant difference for CD117 positive and CD117 negative patients(P=0.290).(2)The level of serum calcium,blood sedimentation and proportion of plasma cells of CD117 positive were higher than that of CD117 negative,while there were no statistical difference in hemoglobin,albumin,creatinine,beta 2-micro globulin,lactate dehydrogenase,c-reactive protein,calkaline phosphatase between two groups.Conclusion:1.Multiple Myeloma has a predilection on patients of aged over 60 and it affects more men than woman.The most common type was Ig G,the stage III was the most in MM patients.2.Multiple factor analysis showed that hemoglobin,beta2-microglobulin,lactate dehydrogenase had affect on the prognosis.3.The expression of immune phenotype has nothing to do with age,clinical stage and classification.4.The prognosis of CD56 negative patients was worse than that of CD56 positive,and its expression has nothing to do with the clinical indicators.5.The level of serum calcium,blood sedimentation and proportion of plasma cells of CD117 positive were higher than that of CD117 negative.
