Objectives:Investigated and discussed the expression and clinical significance of plasminogen activator inhibitor-1(PAI-1) and von Willebrand factor(v WF) in children with type 1diabetes mellitus(T1DM).Methods:Collected 50 patients who were newly- diagnosed with T1 DM from December 2014 to June 2015 in Endocrinology and Metabolism of Shanxi Children’s Hospital as the T1 DM group, including 28 boys and 22 girls,(8.6 ± 3.7) years. Refer to the diagnostic criteria of World Health Organization(WHO), type 1 diabetes mellitus patients enrolled fasting C-peptide and insulin levels significantly lower than normal, continued to show a flat curve after oral glucose stimulation, excluding type 2 diabetes and other specific types of diabetes, and systemic disease. Choosed 30 healthy children at the same period in Outpatient of Shanxi Children’s Hospital as the control group, 15 boys, 15 girls,(9.2 ± 3.4)years, no diabetes and other systemic diseases, no family history of heart disease and high blood pressure, no history of recent infection and medication. 75 g oral glucose tolerance test(OGTT) fasting plasma glucose <6.1mmol / L, OGTT2 h glucose <7.8mmol / L. The expression changes of PAI-1 in plasma of two groups was measured by enzyme linked immunosorbent assay(ELISA) and compared between two groups and analysed the correlation with haemoglobin A1C(Hb A1C) and fastingbioodglucose(FBG) in the T1 DM group. The same to v WF.Results:The concentrations of plasma PAI-1 and v WF in children with type 1 diabetes mellitus were higher than those of normal controls [(20.339 ± 3.141 vs 16.900 ± 1.801)ng/ml,( 4.900 ± 0.515 vs 3.346 ± 0.645)ng/ml, averagely P < 0.001]. PAI-1 of type 1diabetes mellitus was positively related to the FBG and Hb A1C( r=0.478, P < 0.001;r=0.610, P<0.001), v WF of type 1 diabetes mellitus was positively related to the FBG and Hb A1C( r=0.440, P<0.001; r=0.476, P<0.001).Conclusions:The concentrations of plasma PAI-1 and v WF in T1 DM group were higher than those in control group. PAI-1 and v WF were positively related to the FBG and Hb A1 C in children with type 1 diabetes mellitus. Therefore, plasma PAI-1 and v WF levels can be used as a symbol for reflecting the injury of vascular endothelial function in type 1diabetes mellitus. At the same time, the hyperglycemla state of the children with T1 DM caused endothelial injury, and then led to the increase of PAI-1 and v WF synthesized and secreted by vascular endothelial cell. PAI-1 and v WF will provide new ideas for investigating the pathogenesis of type 1 diabetes mellitus, monitoring and controlling the process of its complications.
