Objective: To investigate the clinical effect between cannulated compression screws and hemiarthroplasty in the treatment of femoral neck fractures(FNF) in the elderly. Through the analysis of clinical data, surgical methods and follow-up results in 79 cases of FNF; then to summarize and compare the safety and efficacy of cannulated compression screw(CCS) and hemiarthroplasty(HA) in the treatment of FNF in the elderly.Methods: From 2012 to 2014,the clinical data of 79 cases of femoral neck fracture in the elderly were analyzed retrospectively. There were 37 patients who were treated by the CCS internal fixation(CCS group), and 42 patients who were treated by the HA(HA group). Comparative analysis of two groups( CCS group and HA group) of the amount of bleeding,operation time, hospitalization time, full weight bearing time, postoperative complications, and to evaluate the excellent rate of hip joint function in 6 months,12 months. All given data was performed by SPSS 19.0 software.Results: All 79 cases were followed-up, at least 12 months( average 13.2 months). Compared with CCS group,the bleeding amount and operation time of patients in the HA group increased significantly(p<0.05); the hospitalization time was not statistically significant(P>0.05); but full weight bearing time was early, postoperative complications were obviously less, the differences of two groups were statistically significant(P<0.05). The excellent rate of postoperative hip joint function( in 6 months) in HA group(95.23%) was significantly higher than that of CCS group(78.38%); The excellent rate of postoperative hip joint function( in 12 months) in HA group(90.48%) was slightly higher than that of CCS group(70.27%); the difference in the hip joint function was statistically significant(P<0.05).Conclusion: The HA in the treatment of femoral neck fracture can effectively shorten the bed time,and has low rate of complications; in addition, there are the advantages of hip joint function in 6,12 months. For the elderly femoral neck fracture, HA has more advantages than the CCS internal fixation, can be used as a preferred mode of operation.
