Objective: The study was to observe the metabolic characteristics of cerebral alveolar echinococcosis and cerebral tuberculoma by using MRS. Research on the value of MRS in the diagnosis and differential diagnosis of cerebral alveolar echinococcosis and cerebral tuberculoma.Methods: From January 2013 to January 2016, we select 12 cases of cerebral alveolar echinococcosis and 10 cases of cerebral tuberculoma diagnosed by pathology or clinical follow-up in our hospital. We observed the changes of MRI and MRS in two groups before surgical or medical treatment. The difference of the positive rate of Lip and Cho/Cr, NAA/Cr, Cho/NAA in the two groups were statistically analyzed with the software of Graph Pad Prism5.Results:(1)MRI of cerebral alveolar echinococcosis and cerebral tuberculoma all showed intracranial nodular enhancement or irregular ring like enhancement and obvious perifocal edema.(2)In 12 patients with cerebral alveolar echinococcosis, the peaks of NAA, Cho and Cr were all significantly reduced, and 9 cases appeared visible Lip peak. Compared with the contralateral normal brain parenchyma, the ratio of NAA/Cr was decreased markedly, whereas Cho/NAA and Cho/Cr increased slightly in the lesions, the differences were statistically significant(P<0.05).(3) In 10 patients with cerebral tuberculoma, NAA and Cr peaks were decreased in different degrees. Cho peak was increased in 8 cases, 6 cases appeared increased Lip peak. Compared with the contralateral normal brain tissue, the lesions showed that NAA/Cr decreased and the difference was not statistically significant(P>0.05), whereas Cho/Cr, Cho/NAA increased and the differences were statistically significant(P< 0.05).(4) The differences of the Lip peak positive rate and the ratios of NAA / Cr and Cho / NAA between the two groups of disease all have no statistical significance(P> 0.05), but the difference of Cho/Cr between the two groups of disease has significant difference(P<0.05).Conclusion:(1)Cerebral alveolar echinococcosis and cerebral tuberculoma have their own characteristics of MRS. As for the diagnosis and differential diagnosis of both diseases, the ratio of Cho /Cr has the largest reference value.
