Risk Factors Of Ruptured Intracranial Aneurysms: A Clinical Study

Objective:To investigate the correlation between aneurysm diameter, blood pressure, neck width, gender, age, smoking, alcohol and the rupture risk of intracranial aneurysms.Methods:Retrospective analysis of the clinical and radiological data of 928 patients with intracranial aneurysm in Department of Interventional Neuroradiology, Beijing Tiantan Hospital between January 2011 and December 2012, divide them into ruptured group(411cases)and unruptured group(517cases) according to wether ruptured, univariate analysis and logistic regression model was used for analysis aneurysm diameter, blood pressure, aneurysm neck width, gender, age, smoking, alcohol.Result:In this study, three hundred and forty-one male cases, 587 female cases. Four hundred and eleven rupturedaneurysms patients, age rang from 14 to 84 years, mean(54.9 ± 11.5) years, 517 unruptured aneurysmspatients, age from 24 to 77 years, mean(54.0 ± 12.3) years. Artery aneurysm site distribution: 179 aneurysms in segment ophthalmic artery, 48 aneurysms in basilar artery, 112 aneurysms in vertebral artery dissection, 223 aneurysms in posterior communicating artery, 91 aneurysmsin anterior communicating artery, 28 aneurysmsin internal carotid artery segment, 24 aneurysms in anterior cerebral artery, 47 aneurysms inmiddle cerebral artery, 78 in aneurysms posterior cerebral artery, 72 aneurysms in cavernous artery, 11 aneurysms in anterior choroidal artery segment, eight aneurysms insuperior cerebellar artery, in 7 aneurysms in petrous artery.Univariate analysis showed that there were statistical significance between the rupture risk of intracranial aneurysms and aneurysm diameter, blood pressure, aneurysm neck width, gender, smoking(p<0.05),there are no statistical significance between the rupture risk of intracranial aneurysms and age, alcohol. Multivariate logistic regression analysis showed that there are statistical significance between the rupture risk of intracranial aneurysms and aneurysm diameter(p<0.05), there are no statistical significance between the rupture risk of intracranial aneurysms and blood pressure, aneurysm neck width, gender, age, smoking, alcohol, but hypertension, gender, and smoking in patients with aneurysm rupture risk is still relatively high.Conclusions:Aneurysm diameter is a key risk of rupture for intracranial aneurysms, and this study did not found that blood pressure, aneurysm neck width, sex, age, smoking, alcohol consumption were association with intracranial aneurysm rupture.
