| Objective:To review the effect of sleeve gastrectomy(SG) and Roux-en-Y gastric bypass(RYGB) for the treatment of obesity and type 2 diabetes(T2DM).Methods : Electronic literature search was performed on Pub Med, EMBASE,Cochrane Library, CNKI and Wanfang database from inception to October 1, 2015.The following keywords were used: “bariatric surgery”, “T2DM”, “gastric bypass”,“sleeve gastrectomy”, “代谢手术”, “减重手术”, “糖尿病”, “胃转流术”, “胃旁路术”,“袖状胃切除术”. Data extraction and quality evaluation of literatures were done by 2researchers independently. The statistical analysis was performed by Review Manager Version5.2. We used mean difference(MD) and standardized mean difference(SMD)to calculate the continuous variables, while rate ratio(RR) and odds ratio(OR) was applied to perform the statistical analysis for dichotomous variables. I2 was used to evaluate the heterogeneity.Result:Finally we got 24 literatures focused on the comparison of the effect of sleeve gastrectomy and Roux-en-Y gastric bypass for the treatment of obesity and type 2diabetes to review. Among all the 24 included studies, a total of 6398 cases were reported. 1 year after the surgery, the remission rate of T2 DM and OSAS were of no difference between the two types of bariatric surgeries(T2DM RR=0.93, 95%CI:0.85-1.01, P>0.05) and(OSAS RR=1.01, 95%CI: 0.93-1.09, P>0.05). Patients receiving RYGB had significantly higher %EWL, remission rate of the remission rate of hypertension and the complications of the surgery compared with those receiving SG(%EWL MD=-9.38, 95%CI:-12.75--6.01, P<0.01),(hypertension RR=0.84,95%CI: 0.74-0.95, P<0.01) and(complications RR=0.62, 95%CI: 0.41–0.95, P<0.05).Conclusion:The sleeve gastrectomy could be a lternatives way for the treatment of obesity and type 2 diabetes. |
