Abstract/Summary:
ObjectiveTo explore characteristics of symptoms, tongue and pulse, blood lipid, homocysteine, carotid atherosclerosis of high-risk group of stroke, and the regulation of Traditional Chinese Medicine(TCM) syndromes, and laying foundation on further study of differential treatment system of high-risk groups of stroke; then we research the correlation between TCM syndromes and blood lipid, carotid atherosclerosis and homocysteine.MethodsReference the medical colleges “diagnostics of TCM 5th Edition” and “Traditional Chinese Medicine terms of clinical diagnosis and treatment- part syndrome” issued by National Bureau of Technical Supervision, we developed a TCM questionnaire, containing symptoms, signs, tongue and pulse, with the questionnaire we observed 398 cases of high-risk groups of stroke. Using SPSS17.0 software, the characteristics of TCM symptoms, tongue, pulse, blood lipid, homocysteine, carotid atherosclerosis distribution in different gender and age were observed. Cluster analysis and summarized the TCM syndromes, then research the correlation between TCM syndromes and blood lipid, homocysteine and carotid atherosclerosis.Results1 The result of TCM symptoms, tongue and pulse, blood lipid, homocysteine, and carotid atherosclerosis analysisThe following appeared in a high frequency: spiritlessness, fatigue, shortness of breath, awkwardness, dizziness, irritability, chest tightness, insomnia, anorexia, dark tongue, white thin moss, stringy and slippery pulse, white and greasy moss, yellow and greasy moss. The detection rate of blood lipid, homocysteine, and carotid atherosclerosis were high. Of those, the detection rate of carotid atherosclerosis was the highest, followed by homocysteine. The detection rate of total cholesterol and triglyceride were higher in women than that in men(P<0.05,P<0.01).The detection rate of total cholesterol in middle-aged group was higher than the elderly group(P<0.01). The detection rate of carotid atherosclerosis in the elderly group was higher than the middle-aged group(P<0.01).2 The result of cluster analysisThe traditional Chinese medicine symptoms of high-risk group of stroke were cluster into 4 types with cluster analysis: heart and spleen deficiency syndrome(31.91%), liver qi and phlegm stasis syndrome(24.37%), blood stagnant syndrome(19.35%), liver and kidney yin deficiency syndrome(16.08%).3 The research between TCM syndromes and blood lipid and homocysteineThe detection rate of blood lipid was higher in blood stagnant syndrome than that in others, and the detection rate of homocysteine was higher in liver qi and phlegm stasis syndrome than others. The statistic results show that the difference of blood lipid and homocysteine between syndromes were not obvious.Conclusion1 In high-risk group of stroke patients, the following appeared with high frequencies: spiritlessness, fatigue, shortness of breath, awkwardness, dizziness, irritability, chest tightness, insomnia, anorexia, dark tongue, white thin moss, stringy and slippery pulse, white and greasy moss, yellow and greasy moss. Patients should take stroke screening, when they have these symptoms.2 The traditional Chinese medicine syndromes of high-risk group of stroke were very complex. These patients could be divided into 4 types with cluster analysis: heart and spleen deficiency syndrome, liver qi and phlegm stasis syndrome, blood stagnant syndrome, liver and kidney yin deficiency syndrome.3 High-risk group of stroke patients should be taken care of homocysteine, especially for liver qi and phlegm stasis syndrome patients. For female patients, their total cholesterol and triglyceride should be given attention especially for blood stagnant syndrome patients. For elderly patients, we should also examine their cholesterol and carotid atherosclerosis.
