Application Effect Evaluation Of Clinical Pathway Management

Clinical pathway(CP) is one of the important tasks of public hospital reform. With the reform of medicare payment methods,it has been received widespread concern in enhancing the quality of medical care and controlling the medical costs. It is worth considering to how to evaluate the effects of the application of clinical pathway and explore the future development of clinical pathway. ObjectivesTo conduct a comprehensive evaluation from four aspects of application effect of patient satisfaction, medical quality, medical work efficiency and economic effect of the clinical path management mode by using a tertiary hospital as a case. Explore the effect of clinical pathway in the sample hospital, and find the problems and measures in clinical pathway management, so as to put forward suggestions for better implementation of clinical pathway, and promote the implementation of clinical pathway. MethodsTo understand the development of clinical pathway both at home and abroad and select indexes which need to be evaluated through literature review and secondary sources; Use surveys to understand the patient satisfaction level in the groups both entering into the clinical pathway and not entering into the pathway. This survey received 906 valid questionnaires which enter into clinical pathway and 509 valid questionnaires which didn’t enter into clinical pathway. This research extract related data of medical quality, medical work efficiency, economic effects through hospital information system, which compares data both in the clinical pathway group and not in the clinical pathway group form the patient satisfaction, medical quality, medical work efficiency and the economic effect. It selects 10 diseases in this aspect and evaluate 4269 cases of patients from 13 indexes. By interviewing key people of sample hospital to learn more problems in clinical pathway management and get some suggestions in the future development. ResultsOverall, the application of clinical path management model has achieved some effects. The patients’ satisfaction has been improved a lot. Improving the medical level of doctors and nurses and helping patients in hospitals are the key to improve the satisfaction of patients in clinical pathway. Reasonable length of hospital stay and checking appointment time are of great significance to improve work efficiency. Strengthening the communication between doctors and patients and human care, and improving the level of service to enhance patient satisfaction also have important significance. The effects are relatively obvious promoted after using clinical pathway in medical work quality. Rehospitalization rate promoted a lot, and some diseases cure rate have increased. The disease mortality rate, hospital infection rate are slightly improved. From the point of view of economic effect, the total cost of single disease didn’t rise. And medicine fee, the proportion of medicine fee, Inspection fee,the proportion of inspection fee have been controlled,consumable cost and its proportion didn’t get good effects. ConclusionsClinical pathway can have good effect on the efficiency of medical work efficiency, which can also enhance the patients’ satisfaction, medical work quality and control medical cost. Thus, this research considers that the application of clinical path management mode of sample hospitals has made some achievements. SuggestionsThe development of clinical pathway needs to rely on the further reform of the payment. The development of clinical pathway needs to rely on information system. The coordination of clinical departments is of vital importance to the development of clinical pathway. Functional departments need to take the corresponding administrative supervision. Improving the level of medical services of doctors and nurses is critical to patient satisfaction.
