Background:Acute pancreatitis(AP) is a common critical disease in the clinic which includes Mild Acute Pancreatitis(MAP) and Severe Acute Pancreatitis(SAP).The latter one is often accompanied by serious complications which cause Multiple Organ Dysfunction(MODS) and then lead to Multiple Organ Failure(MOF),even Shock or death.The mechanism of AP is unknown yet.The studies at present shows the mechanism of AP based on the interaction of a variety of factors and parts.It is significant to probe into and make clear the mechanism of the AP and find the“achieve many things at one stoke” therapeutic measures for the treating of AP and reducing the death rate of SAP.During many years of clinical application of Nan Kai Hospital,it has been demonstrated that Qingyi grains has a function of multi center,multi target and comprehensive regulation or the “achieve many things at one stoke”characteristics for the treatment of AP.Oingyi grains has exact curative effects for AP and could modify the prognosis of AP patients dramatically. So it is meaningful to do a further research of the multi center,multi target and comprehensive regulation mechanism of Qingyi grains for the treatment of AP.This study discussed the relative mechanism of Qingyi grains for treating AP and MOF by animal experiment in order to standardize and popularize the clinical application of Qingyi grains.Objective:To discuss the mechanism of Qingyi grains for treating AP by reducing the inflammatory reaction in rats vivo, modifying the ultra microstructure and ATⅡinjury of pancreatic tissues,injury of lungs and permeability of intestinal tract. It would provide a theoretical basis for treating AP with Qingyi grains.Method:To divide 70 male Sprague-Dawley(SD) weighing 200-240 g in to 3 groups:normal control group(10 rats),AP model group(30 rats) and drug test group(30rats).There was no any treatment of normal control group.2% sodium taurocholate was injected from the retrograde direction into the pancreatic duct of the rats in AP model group and drug test group for inducing AP models.12 hours after the operation,every rats in AP model group was given 5ml normal saline by gavage and 5ml normal saline was given every 12 hours after that.Every rat in drug test group was given 5mldecoction of Qingyi grains(contains crude drug 1g/ml) by garage and then 5ml decoction was given repeatedly every 12 hours. Rats in every group were tested the blood serum amylase(AMY)at the 12 h,24h,48 h after the operation and were observed the pathomorphology changes of pancreas,lung tissues and duodenum through light transmitting mirror and electron microscope. The level of IL-6 and IL-10 in the blood of rats was tested by ELISA and D-lactate was tested by spectrophotometry.SPSS17.0 was used for the statistical data analysis,measurement data was expressed by( x ±s),enumeration data was expressed by ratio and chi-square test was used.T-test was used for the comparison between two groups. When P<0.05,the difference between two groups was statistical significance and when P<0.01,the difference was more statistical significance.Result:Compared to the normal group,the injury level of pancreas and lung tissues and the permeability of small intestine of rats in AP model group aggravate gradually with the time prolonging.While compared to the AP model group,the pathological injury degree of the pancreas and lung tissues,the injury degree of ultra microstructure of pancreatic tissues and intestinal mucosal barrier alleviate with the time prolonging.The level of AMY, IL-6 and D-lactate decreased gradually,the difference had a statistical significance(P<0.05),the level of IL-10 increased,the difference had a statistical significance(P<0.05).And there was a statistical significance among every groups(P<0.05).Conclusion:Qingyi grains could modify the systemic inflammatory response of AP in rats by decrease the level of IL-6 and increase the level of IL-10 and other relative inflammatory factors.It could also improve the clinical therapeutic effects by modifying mutiple organ dysfunction,such as modifing the injury of pancreas and intestine,reducing the edema of intestine,decreasing the permeability of intestine and protecting intestinal mucosal barrier.
