Dermoscopy is a noninvasive microscopic image analysis techniques for observing invo skin micro-structure,blood vessels and pigment.The image structure and characteristics of the the lower part of the epidermis,the papillary dermis and the deep dermis which is invisible to the naked eye, can be seen by dermoscopy. The skin features has a larger corresponding relations to histopathology of the skin lesion.According to the corresponding relations confirm the sensitivity and specificity of the dermoscopic examination. Currently dermoscopy mainly used in the the diagnosis of pigmentary dermatosis,greatly reducing the previous misdiagnosis rate of the pigmented skin lesions to the naked eye.And noninvasive dermoscopy can enable some patients to avoid being blind biopsy or unnecessary trauma caused by the direct surgical resection. For non-pigmented skin inflammation, dermoscopy can identify and diagnose the skin lesions by studying vascular morphology, distribution patterns, density, and other characteristics.Such research is rarely in our country, not yet a mature standard.Psoriasis is a common chronic inflammatory skin disease and easy to relapse.the incidence is higher of the disease.The diagnosis of the atypical clinical cases rely mainly on pathological diagnosis, the pain is great,and the patient is not easy to accept, So noninvasive examination methods has been more and more recognized by the patients and hospitals.Objective: To investigate the dermoscopic features and its diagnosis significance of each part of psoriasis lesions(trunk, face, scalp, mucous membranes, nails, palm toe).Observing the vesicular changes in skin lesions and its relationship with the the lesion development or fading after effective treatment of the psoriasis vulgarism through the dermoscopy.Dynamic observation of the correlation between vessels changes and lesions in psoriasis vulgarism after effective treatment.Methods: 1.Select 120 cases of psoriasis patients, study its background color, color scales, low magnification and high magnification vascular morphology, vascular pattern and arrangement of blood vessels in the row direction using dermoscopy. and count its frequency. All of the data were confirmed by SPSS10.0 software for statistical analysis, Positive rate of various kinds of images should be statistical treatment, and describes the indicators sensitivity. 2.Select 30 cases of psoriasis patients for continuous treatment, Using dermoscopy to do dynamic continuous observation of vascular changes in skin lesions on 0,1,2,3,4,6 weeks after treatment.And at the same time,erythema, scaling, infiltration are scored and record for the same skin lesion, comparing the relationship between vascular changes in skin lesions and skin changes.Results: 1.Psoriasis vulgarism major dermoscopic features, scalp, trunk and limbs,facial parts mainly on a red background, silvery-white scales, vascular regularly arranged pattern, point / spherical vessels under lower magnification and glomeruloid vessel under high magnification, The scalp area is mainly glomeruloid vessel and atypical vascular under high magnification. mucosal changes has no obvious scales.The palmoplantar parts shows a red background, silvery-white scales, point /spherical vessels linear, evenly, regular distributed along the dermal ridges under lower magnification glomeruloid vessel under high magnification. The nail mainly for nail plate shows multiple small pits, distal onycholysis, splinter hemorrhages,salmon patches. Hyponychium shows deformed vascular irregularly distributed, The proximate nail fold vascular morphology changes has noticeable pattern. 2, The lesional vascular lesions nearly normal, vascular diameter decreases,glomus sectional area smaller than before after effective treatment.b.Skin lesions,relative indicators and vessels, relative indicators 、 the score of skin lesions,relative indicators and the score of vascular glands, relative indicators changes with time : 0-4 weeks increased, 4-6 weeks reduced.c. the score of skin lesions,relative indicators was positively correlated with vessels, relative indicators,R=0.914, p<0.01, which had statistical significance.d.Skin lesions,relative indicators was positively correlated with the score of vascular glands, relative indicators,R=0.894, p<0.01, which had statistical significance.Conclusion:1.The research shows,vascular, scales and other patters of psoriasis vulgarism under dermoscopy, can be used to the identification and clinical diagnosis.2.Psoriasis lesional changes was positively correlated with vascular diameter and glomus sectional area, which suggested the observation of lesional vascular changes can be used to observe psoriasis recovery and drug curative effect.
