TCCD Combine With Color Doppler To Analyse The Intracranial And Extracranial Vascular Lesions And Risk Factors Of Lacunar Infarction

Objective:In this paper, we use TCCD combined with Color Doppler ultrasound to study correlation of arteriosclerosis among the Carotid artery、intracranial artery and ophthalmic artery and analyse the risk factors of the Lacunar infarction.Methods:68 cases of lacunar infarction patients who diagnosed by CT or MRI and 27 cases of control group who did not show abnormal image by CT or MRI. Survey the medical history and laboratory examination of biochemical(Lipid), homocysteine, glycosylated hemoglobin.Using Color Doppler ultrasound to check all study subject’s bilateral common carotid artery, internal carotid artery, external carotid artery by B-Flow, and check the bilateral common carotid artery, internal carotid artery, external carotid artery, ophthalmic artery and central retinal artery by color doppler flow imaging.Using TCCD to checking color doppler flow imaging of bilateral middle cerebral artery.Use Color Doppler and spectra Dopple ultrasonography to measure and record hemodynamic parameter of bilateral internal carotid artery、middle cerebral artery、 ophthalmic artery and central retinal artery.The hemodynamic parameter include peak of systolic velocity(PSV)、end of diastolic velocity(EDV、pulsatility index (PI) resistantce index(RI).The software SPSS 22.0 is employed to analyze the all data.Results:1.Through to compare the lacunar infarction patients with control group,there is not obvious significant on age, gender%body mass index (P≥0.05), the comparing of data such as medical history(High blood pressure, diabetes and smoking history), total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), high-density lipoprotein cholesterol(HDL-C),nonhigh-density lipoprotein cholesterol(NONHDL),glycosylated hemoglobin and homocysteine, there is remarkable significance (P<0.01)。2.The comparison of the lacunar infarction patients with control group on the intima-media thickness of bilateral common carotid artery and Plaque Score is statistically significant(P<0.01),the comparison of intima-media thickness of bilateral common carotid artery is counted by independent-samples T test, the comparison of Plaque Score is counted by rank sum test.3.We comparing the lacunar infarction patients with control group respectively by using independent-samples T test to count the pulsatility index (PI)and resistantce index(RI)of bilateral internal carotid artery, middle cerebral artery, ophtlalmic artery and central retinal artery.We comparing the lacunar infarction patients with control group,there is remarkable significant on the pulsatility index (PI)and resistantce index(RI)of internal carotid artery, middle cerebral artery, ophthalmic artery and central retinal artery. (P<0.05), internal carotid artery and middle cerebral artery has remarkable significance on pulsatility index (PI)and resistantce index(RI)(P<0.01). there is not obvious significant on pulsatility index (PI)and resistantce index(RI) of ophthalmic artery (P>0.05).We study the lacunar infarction patients by using Person correlation test to count the pulsatility index (PI)and resistantce index(RI)of internal carotid artery, middle cerebral artery, ophthalmic artery and central retinal artery.We find correlation among internal carotid artery, middle cerebral artery, ophthalmic artery and central retinal artery on pulsatility index (PI) (P<0.05).except internal carotid artery and ophthalmic artery,internal carotid artery and central retinal artery, other blood vessels are closely related (P<0.01).We find correlation among internal carotid artery, middle cerebral artery, ophthalmic artery and central retinal artery on resistantce index(RI)(P<0.05).except internal carotid artery and central retinal artery, other blood vessels are closely related (P<0.01)4.The comparision the lacunar infarction patients with control group respectively by using independent-samples T test,We find osculatory correlation among medical history(High blood pressure, diabetes and smoking history)、total cholesterol(TC)、 low-density lipoprotein cholesterol(LDL-C)、high-density lipoprotein holesterol(HDL-C)、nonhigh-density lipoprotein cholesterol(NONHDL)、glycosylated hemoglobin、homocysteine、intima-media thickness and Plaque Score (P<0.01) We comparing the whole experiment model by using Logistic regression analysis, nonhigh-density lipoprotein cholesterol、homocysteine and intima-media thickness are the independent risk factor for the Intracranial and extracranial vascular lesions of Lacunar infarction (P<0.05).Conclusions:1.TCCD combine with color Doppler can assese the Intracranial and extracranial arteriosclerosis degree and risk factors which get Lacunar infarction, vascular lesions and in Lacunar infarction2.Intracranial and extracranial atherosclerosis is more comprehensive, Recent and distant period of blood vessels were influenced each other by hemodynamics.we must check the Branch and tributaries blood vessels after find coronary arteries.3.Medical history(High blood pressure, diabetes and smoking history),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),nonhigh-density lipoprotein cholesterol(NONHDL),glycosylated hemoglobin,homocysteine,intima-media thickness and Plaque Score have osculatory correlation.Nonhigh-density lipoprotein cholesterol,homocysteine and intima-media thickness are the independent risk factor for the intracranial and extracranial vascular lesions of Lacunar infarction. Survey and control nonhigh-density lipoprotein cholesterol,homocysteine and intima-media thickness and treat basic disease can reduce of lacunar infarction.
