| Objective:To investigate the expression of inflammatory factors (IL-1β、IL-6、 IL-8、IL-17A、TNF-α) in aqueous humor of patients with diabetic retinopathy.Methods:A non-randomized single-center clinical research methods was adopted. The present study was approved by the Clinical Research Ethics Committee of the First Affiliated Hospital of kunming Medical University and followed the tenets of the Declaration of Helsinki.Informed consent was acquired from all participants before experiment. All cases were diagnosed by Slit lamp microscopic examination、Color fundus photography、Optical coherence tomography(OCT) and Fundus fluorescein angiography(FFA) inthe First Affiliated Hospital of Kunming Medical University from Jan. 2015 to Jan.2016.40 DR patients and 40 without DR patients were divided into two groups:1、The experimental group:20 DR cases with 5 years history of type 2 diabetes mellitus (NPDR 11cases、PDR 9 cases, represented for 5 years DR); 20 DR cases with 10 years history of type 2 diabetes mellitus (NPDR 10cases、PDR 10cases, represented for 10 years DR).2、The control group:20 cataract cases with 5 years history of type 2 diabetes mellitu (represented for 5 years DM); 20 cataract cases with 10 years history of type 2 diabetes mellitu (represented for 10 years DM). All fasting and postprandial 2 hours 6ml peripheral vein blood collected from 80 samples were sent to the Clinical laboratory center of the first affiliated hospital of kunming medical university to detect FBG% HbAlc、2hPG by automatic biochemistry analyzer. All 0.2ml aqueous humor were drawed from 80 samples and placed in -80 degrees freezer(waiting for the detection of inflammatory factors).Results:1. The level of inflammatory factors IL-1β、IL-6、IL-8,、L-17A、TNF-α in aqueous humor of 5 years DR patients were (22.109±0.34) pg/ml、 (86.324±1.54) pg/ml、 (342.782±18.55)pg/ml、(32.751±0.68) pg/ml、 (14.065±0.23) pg/ml respectively; and (11.415±0.25) pg/ml、 (18.221±1.45) pg/ml、(19.009±1.13)pg/ml、(13.875±0.42) pg/ml、 (7.944±0.22) pg/ml for 5 years DM patients; The level of inflammatory factors in the 5 years DR patients are higher than that in the 5 years DM patients, the difference was statistically significant (t=15.43、39.01,27.78、 21.42、19、15; p=0.001、0.001、0.001、0.002、0.000).2. The level of inflammatory factors IL-1β、IL-6、IL-8、IL-17A、TNF-a in aqueous humor of 10 years DR patients were (20.589±0.24) pg/ml、(31.480±1.35) pg/ml、(137.903±3.90) pg/ml、(25.460± 0.59) pg/ml、(11.765±0.18) pg/ml respectively; and (15.845±0.53) pg/ml、(16.080±0.79) pg/ml、(13.662±0.53) pg/ml、 (12.084±0.45) pg/ml、(8.056±0.29) pg/ml for 10 years DM patients;The level of inflammatory factors in the 10 years DR patients are higher than that in the 10 years DM patients, the difference was statistically significant (t=1.011v 10.477、30.598、17.670、11.499; p=0.005、0.000、0.003、0.001、0.001).3.5 years DR patients:The level of inflammatory factors IL-1β、 IL-6、 IL-8, IL-17A、TNF-α in aqueous humor of PDR patients were (24.127 ±0.59)pg/ml、(91.434±2.12)pg/ml、 (384.171±30.83)pg/ml、(35.784 ±0.91) pg/ml、(16.432±0.24) pg/ml respectively; and (18.229±0.50) pg/ml、(81.838±2.33) pg/ml、(300.930±12.46) pg/ml、(31.912± 0.96) pg/mlv(12.826±0.21) pg/ml for NPDR patients; The level of inflammatory factors in the PDR patients are higher than that in the PDR patients, the difference was statistically significant (t=4.985、1.887、2.444、 2.532、13.197;p=0.001、0.002、0.000、0.000、0.001)4.10 years DR patients:The level of inflammatory factors IL-1β、IL-6、 IL-8, IL-17A、TNF-α in aqueous humor of PDR patients were (22.667+ 2.27)pg/ml、(35.987±0.17) pg/ml、(144.437±5.77) pg/ml、(30.352 ±0.90) pg/ml、(12.855±0.20) pg/ml respectively; and (18.293±1.50) pg/ml、(27.190±0.37) pg/ml、(124.369±1.05) pg/ml、(20.568±0.90) pg/ml、(9.670±0.17)pg/ml for NPDR patients; The level of inflammatory factors in the PDR patients are higher than that in the PDR patients, the difference was statistically significant (t=3.137、19.392、3.375、16.707、 67.498; p=0.002、0.000、0.001、0.000、0.000)5. The level of inflammatory factors IL-1β、 IL-6、IL-8、IL-17A、TNF-α in aqueous humor of 5 years DR patients were (22.109±0.34) pg/ml、 (86.324±1.54) pg/ml、(342.782±18.55) pg/ml、(32.751±0.68) pg/ml、 (14.065±0.23) pg/ml; and (20.589±0.24) pg/ml、(31.480±1.35) pg/ml、(137.903±3.90) pg/ml、(25.460±0.59) pg/ml、(11.765±0.18) pg/ml for 10 years DR patients; The level of inflammatory factors in the 5 years DR patients are higher than that in the 10 years DR patients, the difference was statistically significant (t=15.134.28.596、16.068、13.978、 7.768; p=0.000、0.000、0.000、0.000、0.000)6. The level of inflammatory factors IL-1α、IL-6、IL-8、IL-17A、TNF-α in aqueous humor of 5 years DM patients were(11.415±0.25) pg/ml、 (18.221±1.45) pg/ml、(19.009±1.13) pg/ml、(13.875±0.42) pg/ml、 (7.944±0.22) pg/ml respectively;and (15.845±0.53) pg/ml、 (16.080±0.79) pg/ml、(13.662±0.53) pg/ml、(12.084±0.45) pg/ml、 (8.056±0.29) pg/ml for 10 years DM patients, the difference was no statistically significant (t=-8.518,1.175、4.755、3.648、-0.331; p= 0.1200, 0.254、0.360、0.482、0.744)7. The correlatively analysis among inflammatory factors IL-1β、IL-6、IL-8, IL-17A、TNF-α in 5 years DR patients and 10 years DR patients, there was a positive correlation and statistically significant (P<0.05);8. the correlatively analysis among inflammatory factors IL-1β、 IL-6、 IL-8, IL-17A、TNF-α in 5 years DM patients and 10 years DM patients, there was no statistically significant (P>0.05).Conclusions:1. The concentrations of inflammatory factors IL-1β、IL-6、IL-8, IL-17A、TNF-α increased and with a positive correlation in aqueous humor of DR patients, which suggests that they have the same effects in the inflammation mechanism and maybe play a important role in the occurrence of DR..2. The concentrations of inflammatory factors IL-1β、 IL-6、EL-8, IL-17A、TNF-α increased in aqueous humor of PDR patients, which indicates that IL-1β、 IL-6、IL-8,IL-17A、TNF-a may play a role in the progress of DR..3. DR patients with 5 years history of type 2 diabetes mellitus have higher concentration than DR patients with 10 years history of type 2 diabetes mellitus about inflammatory factors IL-1βIL-6、IL-8, IL-17A、TNF-α, which indicates that the higher levels of these inflammatory factors, diabetic retinopathy complications may occur sooner, by detecting these inflammatory factors in in aqueous humor of DM patients, it is contribute to prevention and treatment of diabetic retinopathy early. |
