Objective:This study aimed to explore the the effectiveness of Tansitional telemedicine nursing mode on behaviors(drinking and smoking,re-admission rate) related to metabolic value,self-monitoring,self-efficacy and hospital anxiety and depressing questionaire with type 2 diabetes patients.Methods:This is an exprimental study. From 2014.12 to 2015.5,180 type 2 diabetes patients from endocrinology department Ⅱ of the second affiliated hospital of Knming Midical Uiversity were randomly divided into two groups such as the control group(n=90) and the study group(n=90). Conventional telephone followed-up ntervention was implemented in the conrrol group, and the patients of study group wre cared by tansitional telemedicine nursing mode. For study group, nursesand doctors remind patients to monitor FPG and recieve feedback, remind health education acitivity time and place, provid "doctor call" service. At the begining of the sdudy, both of two groups signed informed consent form, finished physiological examination,filled questionaire(including general infromation, self-management quesionaire, self-efficacy and hospital anxiety questionaire and depressing questionaire), the self-management quesionaire. self-efficacy and hospital anxiety questionaire and depressing questionaire were finished again at the end of the study. Compared two groups on behaviors(drinking and smoking.re-admission rate) related to metabolic value.self-monitoring.self-efficacy and hospital anxiety and depressing questionaire with type 2 diabetes patients.Results:On metabolic value(such as BMI, blood pressure.UA, TG, ALB, TP), there are positively improvement among two groups in FPG and HbAlc(P< 0.05),and siginficatly change of study group was better than control group,but others were no significant differences (P>0.05). there were no diffirence among tow groups in smoking and drinking (P>0.05), the same happened in re-admission rate (P>0.05); On self-management activities, the total scores, diet management, blood glucose monitoring and foot management were increased relative to baseline data in study group(P< 0.05), and had a better improvement on total scores, diet management, blood glucose monitoring scores compared with the control group(P< 0.05),but there is no significant differences on the scores of foot managemt (P> 0.05).neither study group nor control group improve execises management. On self-efficacy part, "diet time","diet quantity", "food choose","hypoglycemia avoid","deal with blood glucose" and "jude illness"were improved positively(P< 0.05), and had a better increased on "diet time", "diet quantity", "food choose","hypoglycemia avoid","deal with blood glucose" compared with the control group(P<0.05). neither study group nor control group improve "exercise" and "no influence in life". On anxiety and depressing feeling, study group could reduce patients with anxiety and(or) depressing feeling (P<0.05)Conclusion:Our data showed that tansitional telemedicine nursing mode could improve FPG, HbAlc, self-management skill, most parts of self-efficacy skills and feeling of anxiety and depressing better than control group; so Tansitional Telemedicine Nursing Mode can be recommended in clinical type 2 diabetes care, but the long-term effectiveness should be test in the following study.
