Background:The incidence rate of traumatic brain injury in our country is higher than before.More than 100 thousand people die for traumatic brain injury every year,which is the most in the world. With the development of modern medicine, especially the rapid improvement of emergency medicine, we can see a big fall in mortality, but levels of coma rise. Coma is the most serious obstacle to awakening, but also the main symptom of severe patients in the Department of Neurosurgery. Coma after traumatic brain injury is generally severe, and is accompanied by secondary brain injury.The recovery of consciousness is slow,and the effect of conventional treatment is poor.Coma patients will easily get worse to persistent vegetative state. And severe brain injury patients with poor prognosis, their mortality and disability rates are very high, even if consciousness is restored, the possibility of neurological dysfunction is still high. Scholars of various countries have conducted a wide range of research on the treatment, but there is still no single effective drug or treatment, which has become a major difficulty. At present scholars believe that a comprehensive rehabilitation therapy has good effect to coma after craniocerebral trauma. The current focus of research abroad is about acupuncture, multi sensory stimulation, electrophysiological and neural stem cell transplantation. Through animal experiments, CT, MRI, MRA and other laboratory tests and clinical efficacy, many scholars find acupuncture treatment, multiple sensory stimulation and electrical stimulation have a stimulating effect of coma patients after TBI.But the treatment and standards are different, acupuncture points and the electrophysiological stimulation parts and frequencies are different, evaluation methods also have differences. The treatment and standards lack systematicness and regularity.Objective:To observe the clinical effect of electroacupuncture at optimal selection of acupoints combined with five sensory stimulation in coma after traumatic brain injury. To explore the effectiveness and possible mechanism of electroacupuncture at optimal selection of acupoints combined with five sensory stimulation in coma after traumatic brain injury.Methods:Using the random number table method, according to the order random in 30 patients with stroke coma, the treatment group of 15 cases,15 cases in the control group,fall off in 1 case. All patients were based on conventional comprehensive treatment.The treatment group received electroacupuncture at optimal selection of acupoints combined with five sensory stimulation. Electroacupuncture one time a day,the weekend break, five sensory stimulation one time every day, the weekend doesn’t break, a total of 4 weeks of treatment.Compared with two groups of patients coma time,awakening rate, Glasgow coma scale after 4 weeks of treatment, Glasgow Outcome Scale, total effective rate, and CBV and CBF in brain damage zone of 256 slice CT brain perfusion images.All data were analyzed with SPSS 19.0 software.Results:Two groups have no statistic differences in age, sex distribution, Glasgow coma scale before treatment.(P>0.05) Awakening rate of the treatment group was 66.67%, awakening rate of the control group was 35.71%.Coma time of the treatment group is shorter than the control group. (P>0.05)The GCS score, GOS score,and the total effective rate of treatment group,were superior to control group, there was significant difference (P<0.05). The two groups after treatment CBV, CBF damage zone comparison teeatment group were higher than control group, groups after treatment, there is significant difference (P<0.05).Conclusion:Based on conventional comprehensive treatment,electroacupuncture at optimal selection of acupoints combined with five sensory stimulation can shorten coma time, improve GCS, GOS score, increase cerebral blood flow, improve microcirculation, promote functional recovery.The treatment has certain effect on promoting awakeness, and played a certain role to improve prognosis of patients, to improve the patient’s quality of life.It has a certain effect on the prognosis of patients, and is helpful to improve the quality of life of patients after trauma brain injury. It’s effective,cheap,and sequela. It has a certain value of clinical research and promotion.
