The Clinlic Observation On The Treatment Of Diabetic Nephropathy In Ⅳ Period With YiQiYangYinQingLiQuFengHuaYu Metheod

Objective:To observe and validate the clinical curative effect on Diabetic nephropathy (IV period) by using the method of replenishing qi nourishing yin,clearing,dispelling wind and removing blood stasis.Methods:Collate the main therapeutic treatment and clinical results that have been obtained by consulting relevant literature and researching Chinese and western medicine etiology and pathogenesis of DN. On the basis of that, summarize and analyze the treatment characteristics on DN patients with stage IV by replenishing qi nourishing yin,clearing,dispelling wind and removing blood stasis, then observe its curative effect.60 outpatients and inpatients are chosen by clinical observation in urology department of the hospital of TCM in Jiangsu. They are randomly divided into treatment group and control group,conventional western medicine treatment is given to control group, while the treatment group is on the basis of this combined with the method of replenishing qi nourishing yin,clearing,dispelling wind and removing blood stasis:Raw radix astragali 30g, yam medicine 10g, mountain cornus 15g, raw radix rehmanniae12g, radix scrophulariae 10g, radix trichosanthis20g, stigmamaydis 15g, fine pyrrosia leaf 30g, orientvine 30g, night excrementum bombycis 20g, winged euonymus twig 20g, purple salvia miltiorrhiza 15g. Depend on the pathogenetic condition to add and subtract its amount. One agent a day, decocting it with water twice, extracting the juice 400 ml and taking warm decoction by twice in the morning and evening. After 12 weeks’treatment, observe the changing rate of TCM symptom score, U-TP, FPG, Scr, BUN, UA, Alb, Hb, ALT, AST before and after treatment, and record it on statistical analysis.Results:The total clinical effective rate was 83.3% in treatment group, the effective rate of TCM symptoms improving was 86.7%; The control group total effective rate was 70%, the effective rate of TCM symptoms improving was 73.4%. Within the group comparison:after treatment, U-TP, FPG, Scr, BUN of the two groups were decreased (P< 0.01); treatment group on UA was significantly improved(P<0.01), while the control group was not. Two groups of Hb, Alb, ALT, AST after treatment had no obvious change.Comparison between groups:on the improvement of U-TP、FPG、Scr、BUN symptoms and signs, the treatment group was superior to control group (P<0.05). There was no significant difference in UA、Alb、Hb、ALT、AST between the two groups after the treatment.Conclusion:The method of replenishing qi nourishing yin,clearing,dispelling wind and removing blood stasis on DN (IV period)of patients belongs to qi-yin deficiency and damp-blood stasis syndrome has good curative effect. The therapy can also improve the clinical symptoms, reduce the proteinuria, control blood sugar and improve renal function, being worth further research.
