ObjectiveTo recognize the effectiveness of traditional Chinese medicine (TCM) in treatment of neuropathic pain through observing the clinical effects of modified Shengjiang powder in treating it. Take it as objects of observation the curcumin, emodin contained in Shengjiang powder and the albiflorin, tetrahydropalmatine which we used added in it, then comparative analyze the analgesic effects that the four herbal monomers have on mice with neuropathic pain, so to provide the experimental steps for further investigation, and to make basic studies for clinical R&D of new-type herbal analgesics.Methods1.We observed the effects of curcumin, emodin, albiflorin and tetrahydropalmatine to the pain behaviors of the CCI model rats:we randomly divided 66 male SD rats into 6 groups:the curcumin group, the emodin group, the tetrahydropalmatine group, the albiflorin group, thepregabalin group and the CCI group,11 rats in each. After modeling successfully on the sixth day following the operation, we gave curcumin 200mg/kg, emodin 100 mg/kg, tetrahydropalmatine 80mg/kg, albiflorin 100mg/kg, pregabalin 15mg/kg to each observation group by intragastric administration and normal saline in the same volume to the CCI group once a day for 14 days. The threshold of mechanical withdrawal and the thermal and painful sensation were detected. The changes of the mechanical and heat hyperalgesia thresholds of the CCI model rats after modeling and after drug administration were observed.2. ELISA method was used to test spinal cord IL-1 β,TNF-a concentration of curcumin, emodin, tetrahydropalmatine, paeoniflorin, CCI model groups:66 male SD rats were divided into the curcumin group, the emodin group,the tetrahydropalmatine group, the paeoniflorin group, the pregabalin group and CCT model group, with 11 rats in each group. At 6d after operation,7d after dosing,20d after dosing, one, three and seven SD rats were taken to test IL-1β, TNF-α concentration in L4-6 spinal cord and to analyze the effect of drug treatment on two inflammatory factors.3. We observed the clinical effect of modified Shengjiang powder in treating neuropathic pain in two cases of the First Hospital Affiliated to Guangzhou University of Chinese Medicine and analyzed the etiology and pathogenesis, the diagnosis and treatment methods and drug applications of it.Results1. Mechanical pain threshold determination results:After operation, the mechanical pain threshold of SD rats decreased significantly and to the lowest level at 5d after operation; after 9d after dosing, the pregabalin group amd the rhubarb group had no evident statistical difference with the normal group, P>0.05, while the rest groups had evident difference with the normal group, P<0.05. Comparing the pain threshold average value, the mechanical threshold value of the pregabalin group amd the rhubarb group were higher than that of CCI model group; at 12d after dosing, the mechanical threshold value of the pregabalin group, the paeoniflorin group, the emodin group, the tetrahydropalmatine group were higher than that of CCI model group while the pain threshold value of the curcumin group,CCI group were lower than that of the normal group, the difference had statistical significance, P< 0.05. Behavior testing results indicated that all groups had the pain-killing effect and their effect contrasts are as follows:pregabalin ≈ emodin> tetrahydropalmatines≈paeoniflorin>curcumin.2. Thermal pain threshold determination results:After operation, the hot-pain threshold value of SD rats decreased evidently and to the lowest at 5d after operation;at 5d after dosing, the hot-pain threshold value of each groups was higher than that before operation;the hot-pain threshold value of the emodin group was evidently higher than that of the curcumin group and the difference had statistical significance P<0.05;at 7d after dosing, the hot-pain threshold value of the pregabalin group and the tetrahydropalmatine group was evidently higher than that of CCI model and the difference had statistical significance P<0.05;at 9d after dosing, the hot-pain threshold value of the pregabalin group, the paeoniflorin group, the curcumin group, the tetrahydropalmatine group was higher than that of CCI mdoel group and the difference had statistical significance P<0.05;at 12d after dosing, the hot-pain threshold value of each group was evidently higher than that of CCI model group and the difference had statistical significance P<0.05. The listed behavior testing results indicated that all monomers of Chinese traditional herbs have pain-killing effect and their effect contrasts are as follows:pregabalin ≈ tetrahydropalmatine> paeoniflorin ≈ curcumin≈ emodin (among them, the effect of emodin may be better than others in the early stage of dosing).3. Detection results of IL-1 β and TNF-α concentration in the spinal cord of rats:at 7d after dosing, IL-10 and TNF-α concentration of the curcumin group, the tetrahydropalmatine group, the pregabalin group were lower than that before dosing;the descend range of IL-1 β and TNF-α from high to low: pregabalin, tetrahydropalmatine, curcumin;pregabalin, curcumin, tetrahydropal matine. At 20d after dosing, the inflammatory factors of all dosing groups were lower than that before dosing and the group with the biggest descend range was the tetrahydropalmatine;for other dosing groups, the descend range of IL-1 β and TNF-α from high to low:pregabalin, emodin, curcumin, paeoniflorin; curcumin, pregabalin, emodin, paeoniflorin; at 7d and 20d after dosing, IL-1 β and TNF-α of CCI model group also decreased and the descend range was even bigger than the other five groups.4. The modified Shengjiang powder mainly for treating pain mental derangement rational medical record, clinical curative effect is obvious.Conclusion1.The animal experimental observation, CCI model rats after mechanical pain threshold, thermal pain threshold decreases significantly, about five to six days after surgery to a minimum extent, constant pain for a long time, so the CCI model rats pain stability is good, pain is good mental derangement rational animal model.2. The modified Shengjiang powder contains-to add flavor and the commonly used drugs of traditional Chinese medicine monomer of curcumin, emodin, tetrahydropalmatine, paeoniflorin can improve CCI model mechanical pain sensitivity and thermal pain sensitivity in rats. The pain-killing effect of emodin and tetrahydropalmatine is similar to that of pregabalin.3. After dosing monomer of Chinese traditional herbs like curcumin and pregabalin, IL-1 β and TNF-α concentration decreased but CCI model group also had the similar decreasing trend. Considering SD rats had been self-healed, so monomer of Chinese traditional herbs may not have evident regulating effect on two inflammatory mediators:IL-1β and TNF-α.4. The modified Shengjiang powder with neuropathy pain effect is remarkable.
