Backgroud and objectives: Chronic nonspecific low back pain(CNLBP) has been a hot and difficult topic in rehabilitation medicine, because of the universality,complexity and severity. The study found that the development of CNLBP had a closed relationship with neuromuscular function, but there was still not a research for it. In addition, whole body vibration(WBV) exercise had a good curative effect in joint pain and neuromuscular function. But, there are only a few research have been finished. So our study prepared for revealing the relationship about lumbar proprioception and muscle strength. And we want to reveal WBV exercise in improving the the lumbar neuromuscular function in patients with CNLBP by a randomized controlled trial. To explore a more effective and convenient CNLBP rehabilitation training method, and provide theory basis for preventing and treating,having a certain theoretical significance and research value.Research 1: Reliability of CON-TREX isokinetic dynamometer test lumbar proprioception1.Methods: According to the exclusion criteria, 25 CNLBP subjects and 25 healthy person have been selected on March 2014 to June 2014. All subjects’ lumbar proprioception were tested by passive reproduction test, and haing a same test after two days. And the retest reliability calculated by intraclass correlation coefficient.2.Result: There were the healthy subjects completed for lumbar proprioception repeatability measurement results: Flexion(ICC=0.768), Extension(ICC=0.700).There were the CNLBP subjects completed for lumbar proprioception repeatability measurement results: Flexion(ICC=0.766), Extension(ICC=0.719).3.Conclusion: The human body strength assessment and training system(PHYSIOMED CON-TREX TP1000; Germany) has a good test reliability in healthy subject and CNLBP subject, It can be used for lumbar proprioception assessment.Research 2: Relevant research of neuromuscular function indexs for patients with chronic low back pain1. Methods: According to the exclusion criteria, 28 CNLBP subjects have been selected on September 2014 to January 2015. And testing the lumbar neuromuscular function indexs(lumbar proprioception, isokinetic strength, static standing balance),to analysis the correlation these lumbar neuromuscular function indexs of patients with CNLBP.2.Result: There was no correlation between lumbar proprioception and isokinetic strength at different speed(All P > 0.05). And having a significant correlation between lumbar proprioception with movement area with colsed eyes(r=0.506,P<0.05).3.Conclusion: There were a correlation between pain and neuromuscular function.But we didn,t find a correlation between proprioception and muscle strength, thisresult remind that we were not only pay attention to core muscle strength training, but also lumbar proprioception, and they both cannot substitute for another. Finally,proprioception training can improve the ability of standing balance for patients with CNLBP.Research 3: The study of whole body vibration inventation on neuromuscular function for patients with chronic low back pain1.Methods: 46 subjects have been selected according to the inclusion criteria on October 2014 to August 2015, and were randomly divided into WBV group and Non-WBV group. Lumbar proprioception, pain, static standing balance and isokinetic strength were tested at before and after intervention training. According to the data,we analyzed that the effect of eight weeks WBV training on neuromuscular function for patients with chronic low back pain.2.Result: WBV training improve lumbar prorioception better than non-WBV group significantly(P<0.05), but have no statistical significance in pain, isokinetic strength and static standing balance(P>0.05). And we found that pain, extension prorioception,extension isokinetic strength and static standing balance at closed eyes improved significantly for comparison in WBV group(all P<0.05).3.Conclusion: As a new type of neuromuscular training method, WBV training can improve the extension proprioception, extension muscle strength and balance at closed eyes, and having a better effect on improving proprioception comparison with regular exercise training. So we suggest that WBV training is a suitable rehabilitation training method for patients with CNLBP.
