OBJECTIVE a To summarize the endoscopic and pathological characteristics of CAG. b To evaluate the predictive value of Modified Operative Link on Gastric Intestinal Metaplasia Assessment (OLGIM) in CAG. c To inquire into the influence of Helicobacter Pylori (Hp) infection on the mucosal lesions of CAG.METHODS The 286 patients who had got pathologically confirmed CAG were selected. Their clinical data, endoscopic records, pathological results and Hp examinations were collected and analysed. The stages of intestinal metaplasia were evaluated by OLGIM method. The role of Hp infection in carcinogenesis of CAG was also studied.RESULTS a 225 of the 286 patients were diagnosed CAG by endoscopy ( 78.67%). b 25 were diagnosed low grade intraepithelial neoplasia (8.47%),14 were diagnosed high grade intraepithelial neoplasia (4.90%),12 early gastric cancer(4.20%) and 8 of them infiltrating carcinoma(2.80%). c The rate of intraepithelial neoplasia or canceration in patients with III-IV stage of OLGIM assessment was obviously higher than that of 0-Ⅱ stage (29.79% comparing with 11.72%, P<0.001,OR 3.194,95%CI 1.716-5.947). Hp infection was not significantly related to the rate of intestinal metaplasia and canceration(P>0.05).CONCLUSION a While the endoscopy is an important method to diagnose and assess CAG, the definition of CAG should rely on the pathology, b OLGIM is an efficient index to assess the risk of canceration of CAG patients, c CAG plays a crucial role in the procedure of gastric canceration. Prospective studies of large samples and long term follow-up were needed to explore the relationship of Hp with the canceration of CAG.
