| Objective:To explore the correlation of Multislice Spiral CT(MSCT)lung volumemeasurement and Pulmonary Function Test(PFT)in the diagnosis of early chronicobstructive pulmonary emphysema. Diagnostic value and advantages anddisadvantages of assessment two methods in early chronic obstructive pulmonaryemphysema.Method:Collected with a clear clinical history of smoking and67patients withchronic bronchitis, according to current clinical diagnosis of chronic obstructivepulmonary emphysema PFT measurement indicators based primarily on the totalresidue ratio(RV/TLC)greater than40%, or emphysema in31patients, less than40%was determined not pass PFT diagnosis of chronic obstructive pulmonary emphysemagroup of36patients, each patient was measured at the end of three days of PFT bothMSCT lung volume measurement. Using SPSS17.0statistical software for dataprocessing, measurement data(x s),said groups of data were compared usingindependent sample t-test, MSCT deep end-expiratory lung volume index volume(Vex), deep inspiratory volume (Vin), deep breathing and deep end expiratoryvolume difference (Vin-Vex), dark and deep end-expiratory volume end-inspiratoryratio (Vex/Vin) indicators were measured with PFT residual volume (RV),totallung capacity (TLC),forced vital capacity (FVC), residual total ratio (RV/TLC)application correlation analysis; MSCT lung volume indicators Vex/Vin and PFTmeasurement indicators RV/TLC by RV/TLC than40%of emphysema diagnosticcriteria, drawn ROC curves assess its diagnostic value.Results:Two sets of data to compare the difference was statistically significant (P<0.05), MSCT lung volume indicators measuring indicators RV Vex and PFTsignificant correlation (r=0.927,P<0.01), Vin and TLC significant correlation (r=0.831,P<0.01), Vex/Vin and RV/TLC existence moderate correlation (r=0.664,P<0.05), ROC curves show PFT measurement indicators RV/TLC line area of0.712,MSCT lung volume indicators Vex/Vin line area of0.826, the early diagnosis of chronic obstructive pulmonary disease than PFT measurement indicators RV/TLC.Conclusions:MSCT lung volume measurement indexes and PFT measurementindicators correlated to changes in pulmonary function response, MSCT lung volumeindicators Vex/Vin indicators measured over PFT RV/TLC in the reaction of chronicobstructive pulmonary emphysema early changes with higher accuracy, so the earlydiagnosis of chronic obstructive pulmonary disease with clinical predictionsignificance, is a more sensitive indicator. Current clinical diagnostic criteria relatedto the lack of MSCT, it is necessary to develop a more in-depth study and lungvolume is more suitable for MSCT early diagnosis of chronic obstructive pulmonaryemphysema standards in order to facilitate more timely and accurate detection anddiagnosis of patients with early chronic obstructive pulmonary emphysema. |
