Objective:To study the clinical efficacy of irreversible electroporation in the treatment of advanced hepatic carcinoma.Subjects:Between July 2015 and September 2015, 8 patients with advanced hepatic carcinoma(10 tumors) were treated by ultrasound-guided irreversible electroporation, using pertacuneous, laparoscope or open surgery in the Rocket Army General Hospital of PLA. Prospectively collect and retrospectively summarize clinical data. Finally, analyse the therapeutic effect of irreversible electroporation.Results:After treatment of irreversible electroporation, the qulity of life scores significantly increased〔(37.75±4.65)scores /(22.25±2.87)scores,P=0.000〕 and serological indicators(TBil, DB, ALT, AST) and tumor marker AFP apparently decreased{(150.40mmol/L / 56.37mmol/L,P=0.046)、(71.60mmol/L / 58.69mmol/L,P=0.012)、〔(52.63±12.14)U/mL /(87.28±27.94)U/mL,P=0.003〕、〔(48.45±13.75)U/mL /(74.40±21.09)U/mL,P=0.000〕、(159.20ng/mL / 82.10ng/mL,P=0.042)}. One patient suffered from persistent upper abdominal pain. No serious complications, such as infection, biliary fistula, hemorrhage, liver or kidney failure, occurred in all 8 patients. Abdominal enhanced CT scanning or MRI after 3 months showed complete ablation in 7 patients and incomplete ablation with some residual in one patient. Eight patients had 6 to 8 months of follow-up(an average of 7.0±0.8 months). During follow-up, all patients had been alive with one case of recurrence.Conclusions:The effect of ultrasound-guided irreversible electroporation in the treatment of advanced hepatic carcinoma was remarkable. Ultrasound-guided irreversible electroporation may deserve clinical application in consideration of its safety and efficacy.
