Objective:To investigate the relationship between preoperative nutritional risk status and postoperative survival rate and complications of pan-creaticoduodenectomy with a view to improve treatment efficacy.Methods:A retrospective analysis from January 2003 to December 2014 of 102 cases with pan-creaticoduodenectomy was analyzed. Twenty factors including eight nutrition indicators may influencing survival were analyzed by univariate analysis andmultivariate Cox regression analysis. Eight nutrition indicators may influencing postoperative complications were analyzed by multivariate logistic regression analysis.Results:The entire group of 102 patients were followed up, The 1-, 3-years survival rates were 76.6% and 26.8% respectively, the median survival time was 16.9 months.47 patients had postoperative complications in grade 2 and above, The rate w-as 46.1%; Among them,21 cases of pancreatic fistula (7 cases of concomitant abdominal infection), the rate was 23.5%; 5 cases of biliary fistula (2 cases of concomitant abdominal infection), the rate was 4.9%; six cases of delayed gastric emptying, the rate was 5.9%; 8 cases of postoperative bleeding, the rate was 7.8%; 16 cases of abdominal infection, the rate was 15.7%; 2 cases of perioperative mortality, mortalitywas 2.0%. Univariate analysis revealed that Preoperative NRS2002 Rating, Onodera’s PNI and albumin were related to prognosis after pancreaticoduodenectomy (P< 0.05), Multivariate analysis identified Onodera’s PNI and NRS2002 Rating were as significant predictors of survival(P<0.05). Multivariate logistic regression analysis showed thatOnodera’s PNI, albumin and body mass index wereas significant predictors of postoperative complications (P<0.05),and also showed that Onodera’s PNI and albuminwereas significant predictors of postoperative pancreatic fistula (P<0.05).Conclusions:Onodera’s PNIand NRS2002 Rating are risk prognostic factors for pancreaticoduo-denectomy;Onodera’s PNI, albumin and body mass index as significantpredictors of postoperative complications; Onodera’s PNI and albumin areas significant predictors of postoperative pancreatic fistula. Before pancreaticod-uodenectomy,weshould evaluate nutritional risk status, for patients with nutritional risk, should be strengthened nutritional support.
