Asymptomatic Urinary Abnormalities: Pathological Analysis

Aim:The purpose of the study was to investigate the clinical sympt-oms and pathology in patients with asymptomatic urinary abnormalities.Methods:A total of 205 in-patients with urinary anomalies were ana-lyzed between January 2005 and December 2015 in Guangxi Medical Un-iversity. They were divided into three groups according to the results of urine test,one group with isolated hematuria, the other group with isolated proteinuria, the third group with hematuria and proteinuria. Reviewed and comparison of three groups with the clinical presentations, laboratory data, pathological lesions and treatment effects.Results:Compared to the group with isolated proteinuria and the gro-upwith hematuria and proteinuria, isolated hematuria is more happened in-female patients. Renal damages occurred in 21.46% of the 205 patients. Serum creatinine in the group with hematuria and proteinuria is higher than that of isolated hematuria patients (P< 0.05). Renal pathology classi- ficationwithin three groups was statistically significant. Comparing with is-olated hematuria patients, mesangial proliferation is increased significantly in the group with hematuria and proteinuria (P< 0.05); compared with the other two groups,the hematuria and proteinuria patients of segmental-glomerular sclerosis significantly increased (P< 0.05),the group with hem-aturia and proteinuria of renal tubular atrophy and interstitial inflammatory cell infiltration is relatively isolated hematuria group increased significantly (P< 0.05), isolated proteinuria group capillary tube wall thickening rate significantly increased compared with the other two groups (P< 0.05). Patients with hematuria merger albuminuria group crescent is forming part. The frequency of immunofluorescence sedimentary type in the group with isolated hematuria and the group of hematuria and proteinuria is 48.71% and 77.70% for IgA deposits respectively. Compared with the other two groups, isolated proteinuria group is higher frequency for IgA deposits ( P< 0.05). This suggests the IgA nephropathy is asymptomatic especially clinical manifestations of patients with abnormal urine test for merger of hematuria proteinuria in patients with common pathological types;At the same time, some patients characterized by focal segmental glomerular scl-erosis,membranous nephropathy and systemic vacuities and other secondary kidney disease, this kind of situation is common in hematuria proteinuria in patients with merger. Follow-up 28 patients for 10 to 42 months,there are no improvements in 9 cases given hormones or immunosuppressive therapy.Conclusions:Patients with asymptomatic urinary abnormalities are cli-nical manifestations of hidden,but some patients have more serious pathol-ogical change or abnormal renal function. The most common pathologicalt-ypes of patients with asymptomatic urinary abnormalities is IgA nephropa-thy.The patients with asymptomatic urinary abnormalities,especially thosep-atients with hematuria and proteinuria, should consider kidney pathological biopsy, and early detect the potential serious kidney disease.
