Objective the aim of this study was to investigate the crucial factors influencing on hypospadias surgical success rate in order to improve its success rate by individualized regimen.Method A retrospective analysis of 166 patients with hypospadia performed urethroplasty surgery in the pediatric surgery department of the First Affiliated Hospital of Guangxi Medical University from January 2013 to June 2015 to assess the success rate and complication of the surgery. The relevant factors affected the surgical success rate were using univariate chi-square and multiple logistic regression to analysis.Result All the groups of totally 166 cases, the surgical success rate was 87.95% and the complication rate was 12.5%. However, in the "first time one staged completed urethroplasty" with 75 patients group and the" first time with multi-staged completed urethroplasty" with 28 patients group, the surgical sucess rate were 90.67% and 89.29%, and the complication rate were 9.33%and 10.71%respectively, whereas in the "re-urethroplasty "with 63 patients group, the surgical sucess rate was 85.71% and the complication rate was14.29%. Statistical analysis showed that:hypospadias success rate was significantly relevant to glans of penis, the position of urethra orifice on the extension penis, and postoperative infection.etc. and was significantly irrelevant to age, degree of curvature of the penis, length of urethral defect, and surgical methods.etc.Conclusion Many factors affect the success rate of hypospadias surgery, so the effectiveness to improve the success rate of surgery was to individualized base on the anatomical and the developmental of penis, meanwhile strengthening perioperative management also very important too.
