Object:To compare the effect of anometer silver antibacterial gel and mupirocin ointment on promoting burn wound healing, we established a stable model of skin II-dgree burn, we treated burn wounds with nanometer silver antibacterial gel and mupirocin ointment respectively, then we observed the wounds healing,infection, inflammatory response, microvessel density.Methods:56 SD rats were made into models of skin II-dgree burn and randomly divided into four groups, nanometer silver antibacterial gel group(A), mupirocin ointment group(B), Vaseline group(C), Verification group(D).Burn wounds deep of D group was assessed at 24 hours after burning by HE. Burn wounds were treated with nanometer silver antibacterial gel group, upirocin ointment group, Vaseline group respectively at 1,2,3,7,11,15,21 days after burning. We also treated burn wounds according to the condition of the wounds, then recorded dressing change times. The square of the burn wounds healing and decrustation were measured on 7,11,15,21 days. SD rats were killed at 3,7,14, 21 days. Tissue and secreta were collected. Cultures were performed of all secreta to assess for bacteria infection. We counted the rate of the wound healing, decrustation and infection. We observed the degree of inflammation and collagenous fiber neogenesis by HE, Masson and calculated the mounts of macrophages and microvessel density by Immuno histochemistry at 3,7,14,21 days after burning.Results:(1) All rats have no obvious symptom. The C group’ wounds were redder and sweller than A, B groups’in early days, A group’wounds was lightest among them. The C group’wounds were obsened secreta below crust and adhered closer with dressing after 11 days. A group form a protective membrane on the wound to protect the wound.(2) The time for wound healing: A group was 21.17±1.72 days, B group was 24.50 ± 1.52 days, C group was 27.83 ± 1.94 days (F=22.14, P=0.001<0.05).(3) The number of wound dressing changes in the time of after three days to the wound healing:A group was batter than C group(p<0.05), the difference has statistically significant, there were no difference between A group and B group(P=0.13>0.05), the same as between B group and C group(P=0.285>0.05).(4) The rate of the wound healing:A group was batter than B, C group at 7 days after burning (p<0.05);Three groups had no difference at 11 days (P>0.05); A,B group was batter than C group at 15 days (p<0.05),there were no difference between A group and B group; There were obvious difference among three groups, A group was best, B group was batter.(5) The rate of the wound decrustation:A group was lower than B,C group at 7 days(P<0.05), B group was lower than C group (P<0.05); C group was higher than A,B group at 11 days (P<0.05), there were no difference between A group and B group.(6) Bacterial infection rates of wound:A, B group of the wound did not detect significant bacterial infection, C group detected bacterial infection after seven days, and with time getting the bacterial infection became more higher, eleven days to the maximum(33.3%), it was decreased after eleven days, The Bacterial infection rate was 22.2%.(7) Histological wound:The wounds were showed similar inflammation in group A and group B of HE, But C group were showed more severed inflammatory response than A,Bgroup. The wounds of HE were showed lot of angiogenesis and obviously inflammatory response at the seven days after injury. At the fourteen days, we discovered dermal collagen fiber connections. A, B group of epithelialization werw more better than C group at the twenty one days. From the Masson staining at the fourteen days after burn, we were seen the new collagen staggered distribution of A,B group, but C group was displayed Collagen structural disorder.(8) Wound macrophage content:The first three days of each group continued to increase since the wound after injury, seven days to the maximum, it was decreased at fourteen days after burn, twenty one days to the lowest. At the three days after injury, A group of MC were greater than B, C groups(P<0.05),the difference has statistically significant. At the twenty one days, Group C was still seen a large number of macrophages.(9) CD34 positive blood vessel density:It was increased at the first three days after burn, seven days to the maximum, it was decreased at fourteen days after burn, twenty one days to the lowest. The more significant changes is A group. At the three days and seven days and fourteen days, A group of MVD were greater than B, C groups, P<0.05, the difference has statistically significant. At twenty one days, C group of MVD were greater than A, B groups, the difference has statistically significant.Conclusions:(1) Nanometer silver antibacterial gel and Mupirocin ointment have good anti-infective effect on the burn wounds.(2)Nanometer silver antibacterial gel compared with mupirocin ointment is more effective in reducing the number of wound dressing changes.(3) Nanometer silver antibacterial gel form a protective membrane on the wound to protect the wound, Prevent iatrogenic injury.(4) Nanometer silver antibacterial gel compared with mupirocin ointment, it can indirect promote wound healing and shorten the healing time.
