Diabetic wound is an impaired wound which is often encountered in clinic, one of the reason was the change of pathologic changes in the wound microenvironment enhanced wound inflammation, then the tissue injuries produced by the excessive inflammation happended, and the apoptosis of fibroblast was enhanced. at the same time, the expression of tumor necrosis factor alpha(TNF-α)was increased in diabetes mellitus . TNF-αcan induce apoptosis of fibroblast in the diabetic wound, then the diabetic wound healing was difficuity. In this research, topical drugs (MEBO, SilvaSorbTM Gel) were administrated to the diabetic rats with deep-partial thickness burn induced by Streptozocin to investigate the positive expression of TNF-αwith immunohistochemistry, to observe the wound infiltration of inflammatory cells with HE Stain, to compare the rate of infection and healing of the wound. The percent of wound healing and infection in total wound was calculated. The results :the positive cells of TNF-αexpression in control group were more than the ones in MEBO and SilvaSorbTM Gel(p<0.01), the positive cells of TNF-αexpression in SilvaSorbTM Gel group were less than the ones in MEBO (p<0.05). The rate of wound healing in control group were less than the rate in MEBO and SilvaSorbTM Gel(p<0.01).the rate of wound healing in SilvaSorbTM Gel group were superior to the rate in MEBO group(p<0.05).the rate of contamination in SilvaSorbTM Gel group was less than the rate in MEBO group(p<0.05), the number of positive cells of TNF-αexpression had a good negative correlation with the rate of wound healing(p <0.05).the conclusion: application of topical drugs MEBO and SilvaSorbTM Gel both can promote the rate of woung healing of rat with deep-partial thickness burn, the both group showed singnificant diffefence with the control group (P <0.01);in the wound of diabetic rat, the number of positive cells of TNF-αexpression was reduced, the rate of wound healing was increased and the rate of wound contamination was reduced , p<0.05, by administration with MEBO and SilvaSorbTM Gel, and the effect of SilvaSorbTM Gel is better than the effect MEBO in the treatment of chronic diabetic wound ;considered together, these results provided a indication that topical drug (MEBO, SilvaSorbTM Gel) can improve growing of the granulation tissue , then the wound healing was promoted, the reason of it was may be that topical drug reduce the expression of TNF-αin the wound, then promote the diabetic wound healing.
