The granulomatous mastitis, (GM) is not common in clinical, but in recent years, the incidence has been gradually increased. Treatment is difficult, abscess is often recurrent, and even formate fistula and delayed healing. Studies have shown that the causes of this disease are multifarious, including bacterial infection, autoimmune factors, high prolactin level, etc., but its specific pathogenesis and mechanism is not yet clear. Although it is a kind of inflammatory diseases in breast,the effect of conventional anti-inflammatory treatment is poor, the choice of surgical approach also has some difficulties, some patients in multiple operation still relapse, eventually leading to mastectomy,Although the disease does not pose a threat to the patient’s life, but seriously affect the patient’s breast shape, and give patients a great impact on life, to bring great psychological burden and physical and psychological harm to patients.Itcan be said to be "benign disease, malignant behavior.Because the disease is mostly manifested as breast lump, and the lump bringing it is medium or hard texture, irregular shape, the border is not clear, involving the skin, often misdiagnosed as breast cancer. At present, this disease is often treated with steroid therapy, but also some people put forward the antibiotic therapy, but the treatment effect is not good. As clinical understanding of the disease gradually deepened, the gradual development of the concept of therapy and drug, the clinic has a variety of treatment strategies can be chosen, such as traditional Chinese medicine and Western medicine combined treatment, simple tumor resection,steroid therapy, expanding resection are effective treatment, even though there is no standard treatment for this disease.To figure out the pathogenesis of GM and standard treat against the disease has great and far-reaching significance.Objective (s):To explore the effect of different methods of treatment of granulomatous mastitisMethods:Retrospectively analyzing the clinical material of 273 cases with granuloma mastitis in the hospital from November 2009 to November 2014. We separated patients into diffient groups according to take antibiotic therapy, steroid therapy, surgical treatment, surgery and steroid therapy, surgery plus antibiotics, antibiotic and steroid therapy, antibiotic and steroid plus surgical treatment, Comparise the efficiency and recurrence rate of different groups to figure out the best treatment strategy.Results:Group A (antibiotic therapy):12 of 39 patients got rehabilitation after taken antibiotic treatment, including 9 cases of 12 recurrence, the shortest recurrence time was 25 days, the longest recurrence time was 132 days of rehabilitation,effective rate 30.77%,recurrence rate 75%;Group B (steroid therapy):36 patients take steroid therapy and 21 cases get rehabilitation later,12 patients of 21 had recurrence, the shortest time to relapse was 49 days, the longest recurrence time was 332 days,effective rate 58.33%,recurrence rate 57.14%; Group C (surgical treatment): 41 cases took surgical treatment,39 patients with tumors smaller or disappear,27 cases of 39 recurrence, the shortest recurrence time was 23 days, the longest recurrence time was 403 days, the rest of the patients had no recurrence,effective rate 95.12%,recurrenc rate 69.23%;Group D (antibiotic+steroid therapy):40 patients took antibiotics+steroid therapy,32 cases got rehabilitation, including 17 cases of recurrence, the recurrence of short was 79 days, the longest recurrence time was 211 days,effective rate 80%,recurrence rate 53.13%; Group E (surgery+antibiotic therapy):38 patients took the postoperative antibiotics or preoperative antibiotics,37 patients recovered,20 cases of 37 patients with recurrence, the shortest time to recurrence was 103 days, the longest recurrence time was 495days,effective rate 97.37%,recurrence rate 54.05%;Group F (surgery+steroid therapy):39 patients took the steroid after surgery or steroid treatment before surgery,38 cases were cured,11 cases were recurrence, the shortest recurrence time was 103 days, the longest recurrence time was 495 days,effective rate 97.43%,recurrence rate 28.95%; Group G (surgery+steroid+antibiotic therapy):take surgery plus steroid and antibiotic three combination therapy,40 patients of 40 cases recovered, but there are still 5 cases had a recurrence, the shortest time to recurrence was 119 days, the longest time to recurrence was 532 days, the rest of the patients had no recurrence,effective rate 100%,recurrence rate 12.5%.Conclusion(s):For patients of granulomatous mastitis, a variety of treatment methods, antibiotic therapy, steroid therapy, surgical treatment, surgery and steroid treatment, surgery plus antibiotics, antibiotic and steroid therapy, antibiotic and steroid plus surgical treatment more or less treatment effect, and the effect of combined therapy is better than single treatment.
