Objectives To understand sexual features, HIV(Human Immunodeficiency Virus, HIV) infection status and influence factors among college students MSM(men who have sex with men) in Kunming, then to provide valid evidence for HIV intervention and college health education of college students MSM.Methods A cross-sectional study was adopted and collected information with quantitative and qualitative methods. Snowball sampling was mainly used and part of them was from internet in quantitative investigation, data was collected about demographic characteristics, sexual characteristics, and HIV infected among college students MSM, then to have a comparison with sentinel surveillance data of non-students MSM. Qualitative interview was conducted with objective sampling, Negative and sexual orientation and influence factors about HIV infection among college objects during interview. After investigation, HIV blood or saliva screening tests were conducted.ResultsAmong 635 MSM surveyed,160 were college students MSM,475 were non-students MSM.1.Sexual behavior characteristics between college students and non-students among MSM.1.1 First sexual behaviorThe two groups in the first sex of age≦ 19(χ2=67.48 1,p<0.001),the age for first sex with men(χ2=51.545,p<0.001) and the first MSM behavior age ≦ 20 (χ2=104.008,p<0.001) both had statistical significance(P<0.05),The proportion of college students were both higher than non students group.1.2 Latest six months sexual behavior1.2.1 Homosexual anal sex:The two groups in gay anal sex(χ2=30.799,p<0.001), multi-partnering(χ2=10.752,p=0.001), and consist using condoms(χ2=37.871,p=0.001)had statistical significance(p<0.05),the percentage of college students were both lower than non-student among MSM. The two groups in the latest anal sex have no statistical significance(p=0.051).1.2.2 Gay commercial sex:The two groups were statistical significance in commercial sex(χ2=4.482,p=0.035),college students (0.6%)was lower than non-students (4.0%),they had no statistical significance in MB (p=0.345).1.2.3 Heterosexual sex:The two groups had statistical significance in heterosexual sex(χ2=5.633,p=0.019),college students(3.8%)was lower than non-students(9.7%), they were both no statistical significance about insisting on condom use(p= 1.000) and the latest anal sex of using condom(p=0.650).1.2.4 Other high-risk sexual behavior:The two groups had statistical significance in group sex(χ2=4.821,p=0.042),the college students(7.5%) was higher than non-students (3.4%). They had no statistical difference in foreigner sex behavior (p=0.317).1.2.5 Special drugs:The two groups had statistical significance in Rush (X2=24.886,p<0.05), the percentage of using Rush in college students(50%) was higher than non-students (28.4%).91.3%would use Rush when they have sex, and using condom was83.6%.They had no statistical significance in ice(p=0.280).1.2.6 HIV testing and willingness:If there was a new HIV testing place,88.8%of college students MSM willing to take HIV testing nearby school, of which 98.6% thought the test place should be outside the school rather than inside. The two groups had statistical significance in recent one year HIV testing(χ2=5.594,p=0.022),college students (55.0%) was higher than non-students (44.2%). 2 HIV infection and its influence factors of college students MSM 2.1 The college students and non-students MSM of HIV infection were 6.3%and 9.3%respectively, the two groups had statistical significance in HIV infection was no statistical significance(p=0.257) 2.2 Multivariate logistic regression analysis indicated that the past six months of multi-partnering (0R=24.797,95%CI:1.276-481.763), anal sex role as "0" number (0R=18.732,95%CI:1.069-328.301) were HIV infection risk factors, and Han people (0R=0.012,95%CI:0.001-0.263) was protective factors of HTV infection.3 Interview results among college students MSMAmong college students MSM, the junior students were more frequency of sex behavior than seniors. Most people believed that synthetic drug "Rush" was no harmful. Network was the mainly ways to look sex partners, and someone believed that network was a risk factor of HIV infection. There were fewer HIV positives, so they thought that AIDS was far away from them, and judged someone from their appearance. College students MSM had separation between knowledge and behaviors for the reason of lacking of risk awareness, gay was barely recognized, and not familiar with the HIV infection status, particularly in MSM. Someone needed the knowledge of AIDS prevention, while others thought that psychological support rather than knowledge of the requirements. Among eleven HIV positives, six persons were temporary sexual behaviors, and of which one person used ice drug, another one had unprotected sex after drinking.Conclusions1.There existed higher risk of HIV infection in college students MSM than that non-students during the early sexual, because of they were both younger and lower than non-students in the first sex of age, the age for first sex with men and condom use. Their sex activities focused on freshman and juniors, we should strengthen the early sexual health education and prevention, especially in the college entrance, even more in the middle school.2. College students MSM was higher unprotected sex, special drug used and group sex, the result of separation between knowledge and behaviors was more serious than non-students. We should pay more attention to the psychological support and reduced the risky sexual behavior in college students MSM from now on.3. The risk awareness of HIV infection among college students MSM was poor. In HIV intervention, we should increased the latest data of HIV infection, and using the typical cases combined with network behavior to enhance the risk awareness of HIV infection.4. The rates of HIV infection among college students and non-students MSM were both high, and the influence factors of HIV infected were multiple partners and sexual role, in addition, Han people was the risk factors, which should strength HIV education among college students MSM, especially in the ethnic college students.5. The previous HIV testing of college students was higher than non-students MSM, setting up HIV testing place nearby school accepted significantly higher than that in school hospital, and which could become one of the effective means to expand HIV counseling and testing among college students MSM.
