The Role Of Prevention And Treatment With The Zhi Gancao Decoction To The Postoperative Complications And Hypotension Of The Operation On ACS And The Discussion On The Theory Of TCM

Objectives:Based on the research of thoracic obstruction of Qi and yin deficiency (ACS) undergoing PCI surgery cases, evaluate the prevention and treatment of Zhigancao Decoction to the hypotension after chest pain (ACS) and PCI surgery, explore its mechanism so as to provide reference and basis for future clinical intervention.Methods:60 patients from Traditional Chinese Medicine Hospital of Wuxi City during 01/2015 to 10/2016 were randomly divided into experimental group and treatment group who were diagnosed and hospitalized for acute coronary syndrome and syndrome of deficiency of both qi and Yin, those patients were also in line with the PCI operation. Dose the experimental group with oral Zhigancao Decoction, gather and analyze the changes on statistics of two groups of patients on general situation before PCI, risk factors, TCM syndrome score before and after the surgery, blood pressure, hs- CRP and BNP, left ventricular ejection fraction (LVEF) data.Results:1. Two groups of patients in sex and age were statistically without significant difference, the risk factors were compared between the two groups:hypertension, smoking, diabetes, by X2 test.2. There were no difference between two groups of risk factors in a statistical way. And statistical treatment were compared before and after the syndrome integral, before treatment the integral,showing that it is statistically meaningless before treatment, and after treatment, namely integral comparison, the difference has statistical significance, the treatment group is markedly more effective than the control group.3. preoperative comparison of two groups of TNF alpha, showing that there was no significantly statistical difference between two groups on preoperative TNF-a,before and after the treatment group compared, the control group before and after comparison, in two groups were compared after surgery, two groups after treatment were increased, but showing the treatment group was better.4. The two groups before operation of the NBP comparison, showing that there was no significantly statistical difference between two groups on preoperative BNP, the treatment group comparison before and after treatment in the control group before and after comparison, in two groups were compared, showing BNP in the two groups were significantly decreased, the treatment group BNP decreased more significantly, suggesting increased Zhigancao Decoction to improve heart failure is of more advantages.5. The two groups before operation LVEF comparison, showing that there was no significantly statistical difference between two groups on preoperative LVEF, PCI group before and after treatment, control group before and after PCI, in two groups were compared, in LVEF after PCI in both groups were significantly improved, showing the improvements of treatment group for LVEF was better than that of the control group.6. Preoperative comparison of hs CRP in the two groups, showing that there was no significantly statistical difference between two groups on preoperative hs CRP, before and after the treatment group compared, the control group before and after comparison, in two groups were compared, showing two groups after treatment in CRP levels were significantly increased and after treatment two groups compared statistically no different.7. The two groups preoperative baseline blood pressure compared, statistical differences are no found, suggesting that in the two groups preoperative baseline blood pressure are balanced. Postoperative blood pressure of the two groups using fourfold table exact probability method, prompted the use of Zhigancao Decoction, reducing the incidence of hypotension after PCI.Conclusion1 Based on the western standard PCI surgery, after using Zhigancao Decoction, the clinical curative effect of TCM syndrome, compared with the pure western medicine treatment, increased.2. Zhigancao Decoction can significantly reduce the incidence of deficiency of both qi and Yin type obstruction of PCI in patients with hypotension, mechanism may be related the function of Zhigancao Decoction which is anti myocardial reperfusion injury.
