Objectives:To evaluate if it is necessary to remove the parametrial tissue for all the uterine endometrioid adenocarcinoma with cervical involvement and the correlative clinicopathologic characteristics of parametrial spread in uterine endometrioid adenocarcinoma.Methods:We retrospectively identified 164 individuals who was first treated with surgery between January 2005 and June 2015 in Women’s Hospital, School of Medicine, Zhejiang University. All of the 164 individuals had undergone radical hystercetomy and was confirmed with uterine endometrioid adenocarcinoma and cervical involvement after operation.The clinicopathologic data of the 164 individuals was collected for Further analysis.Results:The age of the 164 patients with cervical involvement range from 29 to 83 with an average of 52.9. Among the 164 individuals,eleven(6.7%) had histopathologically confirmed parametrial spread.By Fisher’s exact test to univariate analysis,parametrial spread was significantly correlated with pelvic node metastases,LVSI in the myometrium,and extrauterine transfer but had nothing to do with age,preoperative CA125 levels,tumor grade,myometrial invasion. Multivariate Logistic regression analysis showed that significant indicator for parametrial spread were pelvic node metastases,LVSI in the myometrium.Conclusions:1. The parametrial spread rate of uterine endometrioid adenocarcinoma with cervical involvement was only 6.7%,so conventional resecting the parametrial tissue of the uterine endometrioid adenocarcinoma with cervical involvement is worth discussing.2.Pelvic node metastases and LVSI in the myometrium were independent indicators for parametrial spread.
