ObjectivesTo create a questionnaire that can evaluate the nurse managers’leadership.Methods:Based on literature review and theoretical guidance of the leadership "five forces model", this study used full advantages of qualitative research, we interviewed nurses, nurse managers, nursing department directors and doctors by purposive sample in Hangzhou, refining and screening different indexes by content system. Select 32 experts with experienced nursing management all over 10 provinces and 20 cities, use Delphi Method to evaluate the importance and feasibility of the indexes. After two rounds of expert consultation, the opinion of the experts was nearly consistent. The SPSS 21.0 was used to analyze experts’ positive coefficient, authority coefficient, variation coefficient and coordination coefficient.Results:After two rounds consultation, the opinion of the experts was nearly consistent. Experts’ positive coefficient is 96.8%, authority coefficient is 0.84; Level 1 scales are: importance 0-0.12, operative 0.08-0.11, Level 2 scales are:importance 0-0.18, operative 0.10-0.19, Level 3 scales are:importance 0.03-0.16, operative 0.08-0.18.The Level 1 scales authorities index 0.14-0.26, Level 1 scales authorities index 0.14-0.26, Level 2 scales authorities index 0.22-0.53, Level 3 scales authorities index 0.17-0.38.In conclusion, we obtain a questionnaire comprised of 14 main scales and 52 subscales.Conclusion:The index system was scientific and utility, and the leadership questionnaire was suitable to the nurse managers.
