ObjectiveHip fracture is one of the common injury in orthopaedics. As a kind of trauma, after hip fracture or operation treatment, the body will produces an inflammatory response. Inflammatory response can bring beneficial physiological and pathological effects on the body. Whereas, on some aspects, such as the immune system or coagulation system, Inflammatory response will make negative effects on them. Thus, it’s important to make a proper plan to assess and control the inflammatory response during the whole perioperative period. The goal of this study is to observe the change of the inflammatory markers under the treatment of promoting blood circulation therapy and explore the effect of promoting blood circulation therapy on inflammatory response after hip fracture of the aged during the perioperative period, offers some reference opinions to control the inflammatory response in clinical work. Another aim of this study is to analyze the correlation between age and the level of inflammatory response, and find out whether the difference of inflammatory markers is correlated with age, and to offer reference to Interpret the value of inflammatory markers of patient in different age.MethodThis study is prospective. Between April 2015 and February 2016, consecutive patients with unilateral femoral neck fracture or intertrochanteric fracture undergoing surgical treatment from the First Affiliated Hospital of Guangzhou University of Chinese Medicine were enrolled in the study. All patients were divided into 2 groups (the promoting blood circulation therapy group and the control group) according to the use of Lishangxiaozhong Oral liquid. The WBC, neutrophile granulocyte proportion, ESR and CRP on the 1st and 4th day after admission and the 1st and 4th day postoperation and the presence of fever during the therapy were recorded. All these data were analyzed by SPSS19.0 to see whether there were differences on the values of inflammatory markers of each test and the change of the values before and after the surgery between the two groups. The correlation between the level of inflammatory markers and age were also analyzed.Result60 consecutive patients were enrolled in this study, and 2 patients were excluded because of infection. Among these patients,30 were treated with promoting blood circulation therapy and 28 were in control. The neutrophile granulocyte proportion, ESR, CRP on the 4th day after admission and the ESR on the 4th day after surgery were significantly lower in the promoting blood circulation therapy group (P<0.05). The elevation of WBC, neutrophile granulocyte proportion, ESR, CRP before surgery and WBC, ESR after surgery were significantly lower in the promoting blood circulation therapy group (P<0.05); the ESR, CRP before surgery had significant correlation with age (P<0.05), and the correlation were all bigger than zero, showing positive correlation.ConclusionPromoting blood circulation therapy is effective in control the inflammatory response after hip fracture in the aged. The level of inflammatory response tends to increase with age growing, and it mainly shows in ESR and CRP pre-operation, So it is suggested that when interpreting these inflammatory markers in the aged(60 to 90 years) with hip fracture, the criterion should be broadened properly in the older patients, While in younger patients, the criterion should not be broadened that much, according to the formula of this study.
