Jiawei Ling Jiao Gou Teng Decoction Combined With No.2 Coloclyster Suppository Treatment Of Severe Hand Foot Mouth Disease Complicated With Encephalitis Clinical Research And Influence On T Lymphocyte Subsets,Cytokines

China CDC infectious diseases surveillance data analysis, the number of 2008-2011 December mainland China foot and mouth disease (HFMD) onset of 5,020,994 cases, including 61,623 cases of severe cases, severe ratio of 123/million. Severe hand, foot and mouth disease is mainly concentrated in the April to July (69.42%); laboratory diagnostic results show that the main pathogen is EV71 type (81.75%), CoxA group 16 (4.52%), other enteroviruses (13.73%); male and female incidence ratio of 1.866:1,1-3 years or less severe cases of the total 91.13% of severe cases, patients with severe HFMD mainly concentrated in southern and central regions.. EV71 strains constituting ratio of type-based, but the overall trend showed a decline in volatility, other enteroviruses fluctuating upward trend, severe, deaths mainly caused by EV71 virus infection [2]. Therefore, to explore effective Integrative Medicine program, as soon as possible mitigation of severe HFMD symptoms and complications and reduce mortality, explore Integrative Medicine HFMD mechanisms is critical objective of the present study.This study was designed to investigate the antelope horn Uncaria decoction enema on the 2nd Joint Treated HFMD clinical efficacy and safety, investigate after Integrative Medicine program interventions, HFMD patients cytokines IL-6, IL-10, IL-17 and T cell subsets, NK cells trend to enrich and improve the Integrative medicine clinical and therapeutic mechanisms HFMD, which has accumulated Chinese medicine for emerging outbreaks of infectious diseases pathogenesis and treatment of knowledge and experience. This paper is divided into two parts: Part I:Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository treatment of severe hand foot mouth disease complicated with encephalitis clinical research Methods:A randomized controlled trial design, met the inclusion criteria hospitalized 88 patients were randomly divided into control group (receiving medical symptomatic and supportive therapy) and 46 cases of the experimental group (receiving medical symptomatic and supportive therapy plus Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository) in 42 cases. Time points:day of admission (day 0), hospitalized 4 days (day 5), outcome was efficient laboratory indicators (blood, liver function, blood glucose, CRP, PCT) and adverse reactions.Result:(1) the experimental group and the control group at baseline comparison:two groups in age, gender, C-reactive protein (CRP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen (BUN), serum creatinine (cr), white blood cell count (WBC), hemoglobin (Hb), platelet (PLT) neutrophil count (N#), procalcitonin (PCT), and other types of viral nucleic acid showed no statistical difference. (P>0.05).(2) experimental and control groups turn critically ill patients of HFMD:two groups of patients with HFMD included 88 patients (experimental group of 42 patients in the control group of 46 patients),4 days of treatment, there was no one cases of transfer of critically ill, no 1 died between the two groups showed no significant difference.(3) treatment, the test group were clinically cured 28 cases, effective in 6 cases, effective in 7 cases,1 case, the total efficiency of 97.62%; control group, clinical cure in 18 cases,17 cases markedly effective in 9 cases,2 cases, The total efficiency of 95.65%. There are differences between the two groups was statistically significant.(4) After the treatment, the test group than the control group in reducing CRP, PCT, WBC and other indicators, the difference between the two groups was statistically significant (P<0.05).(5) Joint Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository in the treatment of severe HFMD time fever, rash subsided time aspect, startle and limb shaking time than the control group, the difference between the two groups was statistically significant (P<0.05).(6) the two groups before and after treatment, no significant side effects and adverse reactions, liver and kidney functions were within the normal range, no significant difference between the two groups before and after treatment in two groups.Conclusion:(1) Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository treatment of severe HFMD have better clinical efficacy for improving patient clinical symptoms and inflammatory markers have obvious advantages.(2) Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository is safe and effective, fewer adverse reactions, worthy of promotion.Part Ⅱ:Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository treatment of severe hand foot mouth disease complicated with encephalitis influence on T lymphocyte subsets, cytokinesMethods:ELISA kit to detect HFMD patients with IL-10, IL-17 level, with Roche E601 automated immunoassay analyzer IL-6 levels, with BD flow cytometry HFMD patients T cell subsets and NK cells in absolute the number of different stages, more disease, IL-6, IL-10, IL-17, T cell subsets, NK cell changes between different treatment groups.Result:(1) After treatment, IL-6 test group, IL-17 levels were significantly lower than the control group between the two groups was statistically significant (P<0.01) difference; IL 10-level slightly higher than the experimental group, but between the two groups the difference was not statistically significant.(2) after treatment, CD3+, CD4+, CD8+ cell counts increased level slightly higher than the experimental group, but the difference between groups was not statistically significant (P> 0.05); while in CD3+, CD4+, CD8+percentage terms, the experimental group the rising level was significantly higher among groups and the difference was statistically significant (P <0.05).(3) Before treatment, the percentage of NK cell count and the two groups were significantly decreased compared with before treatment, but there was no significant difference in the level of decline (P> 0.05).Conclusion:Before and after treatment,treated after pro-inflammatory cytokines in the test group IL-6, IL-17 compared with the control group decreased significantly, CD3+, CD4+ CD8+percentage compared with the control group increased significantly in the test group; flavored Jiawei ling jiao gou teng Decoction combined with No.2 coloclyster suppository in order reduce proinflammatory cytokine levels, and regulate T cell subsets, immune function and promote cell recovery.
