Objective To investigate the effect of oral comprehensive intervention on oral health status and blood glucose control in patients with type 2 diabetes mellitus complicated with periodontal disease.Method 1.87 cases of type Ⅱ diabetes patients with chronic periodontal disease, aged 40--78 years old, average age of 56.5±9.3 years,were randomly divided into experimental group (42 cases) and control group(45cases) record, the two groups were tested by the consistency of baseline(including periodontal index, detection of Porphyromonas gingivalis content of subgingival plaque oral health cognitive level survey, fasting blood glucose and glycosylated hemoglobin level), to ensure that the two groups were included in the case of conditional equilibrium.2. After entering the study, the two groups were carried out oral hygiene education and guided the correct method of brushing the teeth, distributed oral health care products.The experimental group were given periodontal intervention including supragingival scaling and subgingival curettage.3. Respectively in 3 months,6 months,12 months, following index were observed and recorded:periodontal index, detection of Porphyromonas gingivalis content of subgingival plaque, oral health cognitive level survey, fasting blood glucose and glycosylated hemoglobin level.4. SPSS 16.0 statistical software statistics all the data.Independent samples t test and rank sum test statistics periodontal index, Porphyromonas gingivalis (Pg) random primer enzyme chain reaction value of for the detection of subgingival plaque and fasting blood glucose and glycosylated hemoglobin level of the baseline level,3 months,6 months,12 months of two groups, the cognitive level of oral health using chi squared test statistics,, the difference was statistically significant when P<0.05.Result 1. There was no significant difference in the periodontal index, oral health cognitive level, detection of Porphyromonas gingivalis content of subgingival plaque, glycosylated hemoglobin level and fasting blood glucose of baseline level between the two groups (P>0.05); 2. While in 3 months, gingival index, attachment loss and detection of Porphyromonas gingivalis content of subgingival plaque of experimental group had decreased compared with baseline level,the difference was statistically significant between the two groups (P< 0.05); plaque index and bleeding index of experimental group had declined compared with the baseline examination, but the difference had no statistically significance(P>0.05); HbAlc (%) and FBG level had decreased compared with 3 months,the difference between the two groups had no statistical significance (P> 0.05); 3. While in 6 months, gingival index, plaque index, gingival crevicular hemorrhage index, attachment loss of experimental group had decreased compared with three months, in which the difference of gingival index, plaque index and attachment loss ware statistically significant between the two groups(P< 0.05); detection of Porphyromonas gingivalis content of subgingival plaque of experimental group had increased compared with three months, but the difference had no statistical significance between the two groups (P> 0.05); The HbAlc (%) level of experimental group had decreased compared with three months while it was increased in control group,there was statistical significance between the two groups (P< 0.05). The FBG level of experimental group had decreased compared with three months while it was increased in control group. BUT the difference of the two groups had no statistical significance (P> 0.05); 4. While in 12 months, gingival index, plaque index, bleeding index and attachment loss of two groups were higher than those of 6 months, in which the difference of gingival index had statistical significance (P< 0.05); regular oral health examination and awareness rate of experimental group had improved significantly,the difference was statistically significant between the two groups (P< 0.05); detection of Porphyromonas gingivalis content of subgingival plaque of experimental group had increased compared with six months while there was no obvious change in control group,the difference had no statistical significance between the two groups (P> 0.05); The HbAlc (%) level had increased compared with six months while there was no obvious change in control group, but the difference had no statistical significance between the two groups (P> 0.05);The FBG level of experimental group had decreased compared with six months while it had increased in control group. But the difference of the two groups had no statistical significance (P> 0.05);Conclusion 1. Active oral comprehensive intervention can significantly improve the oral health status of the patients with type 2 diabetes mellitus complicated with periodontal disease, also can improve the oral health cognitive level of patients; and it is extent to improve the level of glycosylated hemoglobin in a certain.2. Patients with type 2 diabetes mellitus complicated with periodontal disease should be treated with a long-term and continuous basis therapy.It is suggested that oral health examination is carried out once every six months and the patients should cooperate with doctors to carry out periodontal comprehensive intervention actively.
