ObjectiveThis study was aim to analyze the prevalence of HPV latent infection and its subtypes through taking HPV-DNA test on patients at high risk of HPV infection from STD clinic. A clinical trial was conducted by using drug intervention for HPV latent infection. The trial was aim to observe the clinical efficacy of drug intervention on HPV latent infection and whether the treatment was useful to clear HPV at the state of latent infection in order to prevent CA.MethodsHPV-DNA test was taken in people who visited at dermatology and venereal disease department of Guangdong Provincial Hospital of Chinese Medicine during August 2014 to January 2016. The prevalence of HPV latent infection and subtypes distribution characteristic were analyzed. Totally 61 patients who were diagnosed as HPV latent infection and volunteered for the trial were randomized into two groups. The intervention group included 31 patients and control group included 30 patients. The intervention group was treated with topical Youdujing cream which contained zi cao, ya dan zi, e zhu, and xi xin etc, while the control group received topical 5% imiquimod cream. Patients from both groups were asked to clean up the infectious part at the beginning of the treatment, then used the medicine twice a week for eight weeks. HPV-DNA test was taken both at the beginning and four weeks after the end of treatment.Results1. Total positive rate of HPV-DNA was 27.43% in patients at high risk of HPV infection in venereal disease department, with 27.34% in male patients and 27.59% in female. The highest detection rate of HPV subtypes were HPV6,11,16,18.2. Latent infection of HPV subtypes in male patients were more variable than femal patients. Male patients were more easily infected by two low-risk type HPV6 and HPV 11. Female patients were more often infected by two high-risk type HPV16 and HPV18. Thus routine screening and monitoring of HPV16 and HPV18 infection was necessary for female patients.3. HPV latent infection mainly happened at age20 to 25 and age 26 to 30. The total infection rate was 66.13%. HPV latent infections are mainly composed of simple infection, combined infection are mainly double infection, which are mainly combination of low-risk and high-risk type.4. The clinical efficacy was better for make patients than female, which might due to the differences between male and female physiological structure. At the same time, studies have shown that single type HPV latent infection is easier to be cured than the combined type, and the clinical curative effect of treatment of HPV latent infection had no obvious correlation with age and the subtype.5. After the treatment, the total effective rate was 34.43% with 21 patients turning up with normal result of HPV-DNA text. Eleven patients in the intervention group was clinical cured and ten patients in the control group. Difference between two groups are not statistically significant (P>0.05). It illustrated that the effect of two methods were the same in eliminate HPV latent infection.ConclusionThe prevalence of HPV latent infection is relatively high in venereal disease department, and its subtypes have certain characteristics, there are differences in patiences’age and gender, etc. through the clinical observation this study suggests that the intervention group is similiarly effective with the control group. The Yudujing cream appeared to be safer and more effective, leading less adverse reaction. It was suitable for clinical application.
