| ObjectiveTo explore the correlation between different internal injuries including the basic diseases and pathological constitution and exogenous diseases and the regularities of distribution of TCM syndromes elements and the trend of TCM syndromes factors of the exogenous diseases on the basis of internal injuriesMethodsAdopted the research methods of clinical epidemiology study, patients were divided into 352 cases of internal injuries and 111 cases of non-internal injuries. According to 5 elements of TCM exogenous syndrome:"wind", "cold" "hot", "wet", "dry" and 7 elements of TCM internal syndrome:"Phlegm", "wet", "yu", "qi deficiency", "yin deficiency", "yang deficiency".which were extracted by pattern of syndrome of TCM. And designed the observation list and collected the clinical information of patients,The management and establishment of database were by a software of Epidata3.1, using statistical packages SPSS17.0 for data analysis. Comparison between different groups was done with independent sample T-test and samples rank sum tests.Results1 General informationThis study collected 463 cases patients diagnosed with upper respiratory tract infection cases which from the outpatient in emergency room and tutor clinic in the provincial hospital of Chinese medicine hospitals,185 cases(39.96%) of men, women,278 cases(60.04%), there is no statistically significant difference in gender (P>0.05). Those in the study are divided into two parts, one is internal injuries group which includes 352 examples, the other is non-internal injury group (n= 111), and there are statistically significant difference in the ageã€the average annual coldsã€the onset season the temperature and the time and average annual course(P<0.05).2 The analysis of the internal injury group information.The top-five pick of basic diseases in the internal group were malignant tumor, hypertension, diabetes, chronic pharyngitis, chronic gastritis.The top-five pick of pathologic constitution in the internal group were qi-insufficiency constitution, phlegm-dampness constitution, dampness-heat constitution, yin-insufficiency constitution,yang-insufficiency constitution. the basic diseases of lung were found qi-insufficiency constitution(42.59%), yin-insufficiency constitution(18.52%), Phlegm-dampness constitution(17.59%) more common. the basic diseases of the heart were found qi-insufficiency constitution (29.17%), yin-insufficiency constitution(29.17%), phlegm-dampness constitution(14..58%) more common. The basic diseases of endocrinology were found qi-insufficiency constitution (31.11%), yin-insufficiency constitution(35.56%), phlegm-dampness constitution(11.11%) more common. The basic diseases of digestive system were found qi-insufficiency constitution (43.37%), yin-insufficiency constitution(15.67%), dampness-heat constitution(16.87%) more common.the internal injuries of the kidney were found qi-insufficiency constitution (55.56%), yang-insufficiency constitution(11.11%), phlegm-dampness constitution(22.22%) more common.the internal injuries of the nervous system were found qi-insufficiency constitution (33.33%), yin-insufficiency constitution(33.33%), phlegm-dampness constitution(16.67%) more common. The internal injuries of the hematological system was found qi-insufficiency constitution(100.0%) more common and the internal injuries of the immune system was found qi-insufficiency constitution (80.00%) more common.The correlation analysis between the internal injuries and exogenous febrile disease.The correlation between the different basic diseases and exogenous febrile disease. But the exogenous febrile disease on the basis of these pathologic constitution which included dampness-heat constitution, yin-insufficiency constitution were found wind-heat cold more common and the percentage were 53.06%ã€38.24%.3 Internal injuries groups in different observation points and the relationship between TCM syndrome elements.3.1 The distribution of TCM syndrome elements of all kinds of internal injuries:the proportion of exogenous syndrome elements of lung department internal injuries was as the following order:wind(81.48%)〠cold(77.78%)〠hot(65.74%)〠dry(34.26%)〠wet(14.81%)and the proportion of internal injury syndrome elements was as the following order:qi-deficiency(82.41%), phlegm-dampness (65.74%)〠Yang-deficiency (39.89%)〠Yin-deficiency(34.26%) yu(25.93%)〠wet(23.15%)〠blood-deficiency(15.74%). The proportion of exogenous syndrome elements of heart internal injuries was as the following order:wind(85.42%)〠cold(86.67%)〠hot(54.17%)〠dry(50.00%)〠wet(20.83%) and the proportion of internal injury syndrome elements was as the following order:qi-deficiency(91.67%)ã€phlegm-dampness(77.08%)〠Yang-deficiency (47.92%)〠Yin-deficiency(43.75%)〠wet (31.25%)ã€yu(20.83%)〠blood-deficiency (6.25%). The proportion of exogenous syndrome elements of digestive internal injuries was as the following order:wind(90.36%)〠cold(79.52%)〠hot(50.60%)〠dry(32.53%)〠wet(19.28%) and the proportion of internal injury syndrome elements was as the following order:qi-deficiency(79.52%), phlegm-dampness(67.47%)〠Yang-deficiency(44.58%)〠Yin-deficiency(36.14%)ã€wet (34.94%)〠yu(26.51%)〠blood-deficiency (12.05%). The proportion of exogenous syndrome elements of nervous system internal injuries was as the following order:wind(100.0%)〠hot(66.67%)〠dry(83.33%)〠cold(50.00%)〠wet(50.00%) and the proportion of internal injury syndrome elements was as the following order:qi-deficiency(66.67%), phlegm-dampness(66.67%), Yang-deficiency (66.67%), wet (50.00%)〠Yin-deficiency(33.33%)〠yu(33.33%) blood-deficiency(33.33%).3.2 The exchange of TCM syndrome elements of the internal injuries group at the different observation points.The TCM syndrome elements of internal injuries group were divided into deficiency-syndrome group(48.30%), excess-syndrome group(24.14%) and the sthenia syndrome or sthenia syndrome(27.56%). In the study, the top three precedences of the external syndrome elements of the exogenous diseases were "wind", "cold" and "hot" and the top five precedences of the internal syndrome elements were "phlegm-dampness", "qi-deficiency", "wet", "yang-deficiency" and "yin-deficiency" at the first point. At the second point, the top three precedences of the external syndrome elements were still "wind", "cold" and "hot" and the top five precedences of the internal syndrome elements were "qi-deficiency", "phlegm-dampness", "wet", "yang-deficiency" and "yin-deficiency".as well as the "qi-deficiency", "phlegm-dampness" "yang-deficiency"and"yin-deficiency" were higher than before and the "wind", "cold", "hot" and "wet" were less than before.At the third point, the frequency of most exogenous syndrome elements were less than those at the second point and the top five precedences of the internal syndrome elements were "qi-deficiency", "phlegm-dampness", "yang-deficiency", "wet" and "yin-deficiency".At the fourth point, all the external syndrome elements were disappeared largely and the top five precedences of the internal syndrome elements were still "qi-deficiency", "phlegm-dampness", "yang-deficiency", "wet" and "yin-deficiency". In addition, the frequency of qi-deficiency had been high at the whole course. The internal injuries syndrome elements were extremely complex and most of them had 4 syndrome elements, then 5 syndrome elements and 3 syndrome elements. And at the location of zang-fu, all the three point were basically consistent and the top five precedences of it were lung, spleen, kidney, heart and large intestine. Most of them had 3 disease locations, then 2 disease locations and 4 disease locations.Conclusion1 From the point of general information, internal injuries including basic diseases and pathological constitution was correlated with the exterior syndrome, which mainly reflected in the average annual course of exogenous disease progression, and average annual colds. That was to say, the course and average annual colds of internal injuries patients with underlying diseases or pathological constitution were more than the non-internal injuries group.2 From the point of internal injuries information, the exogenous Febrile disease on the basis of these basic disease which included the pulmonary system internal injuries, heart internal injuries, endocrine department internal injuries, digestive internal injuries, kidney internal injuries, blood internal injuries, the immune system internal injuries, and nervous system internal injuries were found chill cold more common. The correlation between the different pathologic constitution and exogenous febrile disease. The exogenous febrile disease on the basis of these pathologic constitution which included qi-insufficiency constitution, phlegm-dampness constitution, yang-insufficiency constitution and blood-stasis constitution were found chill cold more common and the exogenous febrile disease on the basis of these pathologic constitution which included dampness-heat constitution, yin-insufficiency constitution were found wind-heat cold more common.3 From the relationship between different observation points and TCM syndrome factors in the internal injuries group, for the characteristic of TCM syndrome factors, the syndrome factors of exogenous were found "wind", "cold", "wet" more common and the syndrome factors of internal injuries were found "qi deficiency", "Phlegm", "yang deficiency", "wet", "yu". For the development trend of TCM syndrome factors, Exogenous syndrome elements under the drug intervention showed a trend of gradual decline and internal injury syndrome elements showed upgrade firstly than descending latter tendency. |
