ObjectiveBy observing the Clinical Efficacy of the treatment of Ⅱ Ⅲ-type proximal humeral fractures Between Percutaneous pinning combined with pingle bonesetting manipulation and open reduction and locked plate fixation, to explore the effect of two methods of treatment on postoperative shoulder joint activity, providing a new treatment method and theoretical support in the treatment of proximal humeral fractures.Methods40 patients who were accorded with the inclusion criteria received treatment in Shenzhen Pingle orthopedic hospital from January 2014 to January 2015, then randomly divided into two groups, twenty of them were divided into Percutaneous pinning group combined with pingle bonesetting, the others were divided into LPHP group, treated with open reduction and locking plate internal fixation, then Recorded and compared the operation time, the length of incision, blood losses, the excellent and good rate Of postoperative fracture contraposition, shoulder constant pain score and the excellent to good rate of shoulder constant score after 3,6 months and the humeral inversion angle, finally use the SPSS17.0 software to compare the data.ResultsNo significant statistically difference between the two groups in age, gender, the classification of fracture, lateral distribution and injury combined with shoulder joint dislocation cases, the length of incision and Blood Losses in Percutaneous pinning group combined with pingle bonesetting was significantly less than LPHP group,the operation time and the shoulder joint constant pain score 3 months and 6 months postoperatively was 27.50 ±11.18min,7.20±1.96,3.60±1.09, while the LPHP group is 116±24.36min, 8.50±1.87,4.45±1.14, p<0.05, which has statistical significance;the result of the excellent and good rate of fracture contraposition has a significant differance, the fracture of healing time of Percutaneous pinning group combined with pingle bonesetting is 22.00±2.94w, while the LPHP group is 21.70±3.16w, so there is no statistical differences, the excellent and good rate of shoulder constant score 3,6 months postoperatively and The complication incidence rat was non-statistical significance (P>0.05)ConelusionPercutaneous pinning combined with pingle bonesetting manipulation can achieve similar efficacy compared to Open reduction and locking plate fixation, the fracture healing time and the excellent to good rate of shoulder constant score 3 and 6 months postoperatively has no significant difference, but the excellent to good rate Of fracture and the humeral inversion after 6 months in Percutaneous pinning group combined with pingle bonesetting manipulation was less than the LPHP group. Percutaneous pinning group with pingle bonesetting make it easy to have shorter operative time minimally invasive, less postoperative shoulder pain.
