Objective To detect the cytokins IL-4, IL-17, IFN-r and TNF-a expression levels in peripheral blood of patients with aplastic anemia (AA), MDS, acute myeloid leukemia (AML) and to comparatively analyze the T cellular immune function stations among these kinds of diseases.Method During the time between March 2014 and July 2015, collected the Peripheral Blood from 60 patients and 10 healthy person which were all from the Department of Hematology, Provincial Hospital affiliated to Shandong Uneiversity and Using cytometric bead array method to detect the expression levels of IL-4, IL-17, IFN-r and TNF-a in peripheral blood in 20 cases AA,20 cases MDS,20 cases AML and 10 cases of normal controls and evaluated T cell immune status in these three disease indirectly.Results:1. Compared those with normal control group, IFN-r, TNF-a, IL-17 expression level in AA group were increased (p< 0.05), IL-4 expression was not statistically significant (p>0.05); The expression of IL-4 and IFN-r in MDS group was increased (P< 0.05), and the expression of IL-17 and TNF-a was not significant (P>0.05);IFN-r and IL-17 expression level in MDS-RA group were increased (p< 0.05), although the expression of TNF-a increased, the difference was not statistically significant, IL-4 expression difference was not statistically significant (P>0.05); IL-4 expression were increased in MDS-RAEB and AML groups (p< 0.05), the expression of other cytokines difference were not statistically significant (p>0.05).2. Compared with AA group,4 cytokines in MDS-RA group were no statistical significance(P>0.05), IFN-r, IL-17and TNF-a expression in MDS-RAEB and AML group were decreased(p< 0.05), but IL-4 expression was increased(p< 0.05);3. Compared with MDS-RA, the expression of IFN-r、TNF-a and IL-17 in MDS-RAEB and AML group were decreased (p< 0.05), while IL-4 expression was increased(p< 0.05).4. The expression of 4 cytokines in MDS-RAEB and AML group had no statistical differences (P>0.05).5. Compared with MDS group, the TNF-a expression in AA group decreased with statistical significance (P< 0.05) and other cytokines expression difference was not statistically significant (P> 0.05); TNF-a, IFN-r and IL-17 expression in AML, were increased (P< 0.05),IL-4 expression difference was not statistically significant (P> 0.05). (see table 1-6)Conclusion:1. In AA and MDS patients serum the IFN-r and IL-17 expression were increased, which were represent secreted by Th1 and Th17 cells and were negative hematopoiesis regulation factors, indicating that T cells of Th1/Th2, Th1 7/Treg balance to the deviation of Thl and Th17 direction and T cell immune state of this diseases are similar, for T cell activation and increase the secretion of negative regulatory factors to guide cells apoptosis excessive and inhibit normal hematopoietic stem/progenitor cell proliferation, resulting in low hematopoietic function of bone marrow.2. Differential cell factors expression in MDS-RA and MDS-RAEB are absolutly different, indicating that the T cell immune status between them was not similar.And as that with the progress of the disease, in patients with malignant clone increased and (or) with the further change of the abnormal antigen, so that in the low immune function of the body can not effectively remove tumor cells, resulting the malignant clone got proliferative advantage.3. The cell factors differences expression in AML and MDS-RAEB patients serum were not statistically significant, suggesting that the T cell immune status and exerting the immune inhibition role of Th2 cells were increased, a further declining immune function could promote abnormal clones escaping from the immune surveillance, the abnormal accumulation of tumor cells induced by hematopoietic failure.
