Background:With the rapid population aging, the prevalence of elderly patients with dementia is increasing every year. Lacking of effective treatment for dementia at home and abroad, and poor treatment for the late-stage dementia bring heavy burden to families and society. Therefore, studies at home and abroad have changed the research focus to the early diagnosis and treatment. Mild cognitive impairment is a clinical state between normal aging and dementia. It’s the first stage of dementia symptoms, early intervention is very important to slow down the conversion of MCI to elderly dementia. With the guide of "people-centered" care concept, Art therapy combines the arts and humanities feature, breaking through the limitations of memory training, applying to patients with cognitive impairment, using lines and colors to stimulating the perception in witty and relaxed atmosphere, providing a non-verbal communication channels for patients. The art therapy has caused concern of many scholars. Reminiscence-themed activities is more interesting and easier to be accepted by the elderly. Combining of Art therapy and Reminiscence themed could promote the enthusiasm and expressing of the elderly.Objective:Based on the literature review and a professional perspective of nursing care, combining with MCI elderly physiological, psychological, hobbies, etc. This study use Art Therapy with Reminiscence as theme in elderly with Mild Cognitive Impairment on the basis of the theoretical viewpoint of art therapy and Reminiscence, andEvaluate the effectiveness of the intervention, in order to explore an effective way to delay the course of MCI to dementia.Methods:A total of 59 participants were assigned to an experimental or a control group. The experimental group with 29 subjects received a 45-60 minutes Art therapy mainly about reminiscence three times weekly for 12 weeks. The Motreal Cognitive Assessment, Activity of Daily Living Scale, Chinese version of the neuropsychiatric inventory, World Health Organization Quality of life were used to estimate cognitive function, behavioral and psychological, ability of daily living and quality of life with MCI elderly at baseline, week 6 and week 12.Results:(1)The cognitive function of experimental group has improved, significantly for visual space, naming, attention, executive function and language ability. Compared with the control group, The space and executive function score of experimental group has increased in six weeks which was statistically significant difference(P<0.01); The space and executive of control group decreased in 6 weeks compared with base line(P<0.01) and increased in late 6 weeks,but still lower than that of the experimental group(P<0.01). The named ability score of experimental group increased in 6 weeks compared with base line(P<0.01). The language score of experimental group increased in 12 weeks compared with control group(P< 0.05); The attention score falling in 12 weeks(P< 0.01), the experimental group has no obvious change(P>0.05).(2)The mental behavior symptoms of experimental group has improved, especially depression, anxiety, irritability, and intervention factor and time factor exist interaction(P < 0.01). Depression of experimental group has improved in 6 weeks compared with control group(P<0.01),and the symptom is reduced compared with base line(P<0.01). The anxiety of experimental group reduced in 12 weeks compared with base line(P< 0.01),the irritability also reduced compared with control group(P< 0.05). The irritability symptoms of control group has worsened in 12 weeks(P<0.01).(3)The ability of daily life of experimental group has improved,especially instrumental ability of daily life,such as phoning, shopping, handle money. Among them, the intervention factor and time factor of the ability to handle money, instrumental ability,the ability of daily life exist interaction(P< 0.01). The overall ability of daily life has improved in 6 weeks compared with control group(P< 0.05),the score of phoning, shopping has reduced in 12 weeks compared with control group(P<0.01). The ability of shopping and handle money in 12 weeks has improved compared with base line(P< 0.017).(4)The quality of life of experimental group has improved,especially in mental health.social relationships and surrounding environment, among them, the intervention factor and time factor of the mental health and the surrounding environment exist interaction(P<0.05). The mental health and surrounding environment has increased in 6 weeks compared with control group and base line(P< 0.01); the social relation of experimental group in 12 weeks has improved compared with control group(P<0.01);the mental health of control group in 6 weeks has improved(P< 0.01),the surrounding environment is also improved in 12 weeks(P<0.01), but the two index score is still lower than experimental group(P<0.01).Conclusions:Art Therapy with Reminiscence as theme could effectively improve the cognitive ability,mental behavior symptoms, ability of daily life and quality of life in elderly MCI.
