ObjectiveTo investigate the safety and clinical efficacy of transurethral resection of prostate in high risk benign prostatic hyperplasia patients, summary of perioperative preparation、matters needing attention and handling, clinical experience of intraoperative and postoperative monitoring.MethodsRetrospective analysis of the clinical data of 90 cases of high risk benign prostatic hyperplasia (BPH) in the past 3 years from April 2012 to April. To observe the clinical effect of the operation on high-risk benign prostatic hyperplasia patients.Results Retrospective analysis of 90 cases of benign prostatic hyperplasia patients in the past 3 years in the hospital during the period of hospitalization and follow-up after discharge, the amount of bleeding during operation was measured by transurethral resection of the prostate (transurethral). the amount of bleeding was 50-300ml, average 160ml. The weight of benign hyperplasia of the prostate gland is about 16-56g, average 39g, Transurethral resection of the prostate (transurethral) of the operation,30-100min, average 55min, The average 7.6cmH2O of central venous pressure was measured before and after the operation of the external jugular vein, the average central venous pressure was about 10.55cmH2O in the middle of the operation, the central venous pressure of 8 patients was higher than that of 12cmH2O, and the central venous pressure drop to the normal range after intravenous injection,preoperative routine examination of serum sodium was about 140mmol/l, and the average blood sodium level was about 136mmol/l. The International Prostate Symptom Score (IPSS) in patients with an average of about 25.3 points before surgery was reduced to an average of about 6.8 points during follow-up, Residual urine volume was approximately 26ml before surgery and the average follow-up was 295ml, The maximum urinary flow rate was approximately 26ml/s before surgery, and the mean follow-up was about 7.1ml/s. The pathological results of 90 cases of benign prostatic hyperplasia were benign hyperplasia of prostate,90 patients were safely through perioperative period, and no death occurred. All patients were followed up for 6-24 months after transurethral resection of the prostate (transurethral). The average follow-up was about 12 months, There were 2 cases of false urinary incontinence.by strengthening the functional exercise of pelvic floor muscles (levator ani exercise) significantly improved after one month. Urethral stricture in 4 cases (including 2 cases of membranous urethral stricture,2 cases of urethral stricture), after the operation in a timely manner to the urethra massage (urethral dilation) after the improvement, Postoperative patients and their families were satisfied with the clinical treatment effect.ConclusionTransurethral resection of the prostate for the treatment of high-risk benign prostatic hyperplasia patients is safe and effective.Adequate perioperative preparation, intraoperative combined application of the pubic bladder stoma, low pressure irrigation, strict intraoperative and postoperative monitoring, timely and effective treatment and nursing, as well as individualized surgical procedure is the key to the success of surgery.
