Objective:With the improvement of living standard, the incidence of colorectal cancer increased year by year. To observe the expression level of epidermal growth factor receptor (EGFR) and human epidermal growth factor receptor-2 (HER2) in colorectal cancer (CRC) and discuss the role of EGFR and HER2 in CRC occurrence and development, offer a possible new biomarker for diagnosis and the prognosis of CRC.Methods:1. Immunohistochemical (IHC) was used to detect the expression of tissue EGFR and HER2 in 76 cases of CRC primary lesion and normal tissue adjacent to carcinoma,40 cases of colorectal adenomas. Preoperative serum samples were extracted from 76 cases of CRC and 40 healthy volunteers. Serum levels of EGFR and HER2 were determined by the enzyme-linked immunosorbent assays (ELISA).2. To analysis the relationship of clinicopathological features with the expressions of EGFR and HER2 and the correlation of diagnosis and prognosis with colorectal cancer.3. All data were processed by SPSS 13.0.Results:1. Expression levels of serum EGFR and EGFR1.1 Expression levels of serum EGFR The average levels of EGFR of the serum experimental group (7.3±1.2) mg/L was significantly higher than the serum control group (3.5±1.5) mg/L, there was statistically significant (t=14.845, P<0.001). There were 54 of 76 case of CRC patients and 18 of the normal control group serum EGFR≥6.5 mg/L.The sensitivity of EGFR was (54/76)71.1% and specificity was 55.0%(22/40).1.2 Expression levels of serum HER2 The average levels of HER2 of the serum experimental group (9.5±2.3) mg/L was significantly higher than the serum control group (3.5± 1.2) mg/L, there was statistically significant (t= 15.410,P<0.001). there was statistically significant (P<0.05) There were 68 of 76 case of CRC patients and 23 of the normal control group serum HER2 ≥5.9 mg/L mg/L.The sensitivity of HER2 was (68/76)89.5% and specificity was (17/40)42.5%,2. Expression levels of tissue EGFR and HER2 in CRC2.1 Expression levels of tissue EGFR in CRC The results of IHC showed that the expression of EGFR in cells was mainly in the cell membrane, occasionally in cytoplasm; The expression of tissue EGFR in CRC was higher in adenoma tissue (x2=28.382, P<0.001)and NT(x2=60.674, P<0.001).2.2 Expression levels of tissue HER2 in CRC The expression of tissue HER2 was mainly in the cell membrane, some is also available in the cytoplasm, the expression of tissue HER2 of CRC was significantly higher than NT(x2=30.612, P<0.001), adenoma tissue (x*=8.727, P=0.003),there was statistically significant.3. Correlation in the expression of tissue HER2 and EGFR of CRC and general clinical pathological characteristics3.1 Correlation in the expression of tissue EGFR of CRC and general clinical pathological characteristics The level of EGFR expression in tissue was correlated with lymph node metastasis and Dukes stage (all P<0.05), while without relation with histological type, tumor site, gender,sex, age (all P>0.05). The expression of tissue EGFR of CRC of Dukes C/D and LNM was significantly higher than that of Dukes A/B and no metastasis (both P<0.05).3.2 Correlation in the expression of tissue HER2 of CRC and general clinical pathological characteristics The level of HER2 expression in tissue was correlated with lymph node metastasis and Dukes stage (all P<0.05), while without relation with histological type, tumor site, gender,sex, age (all P>0.05). The expression of tissue HER2 of CRC of Dukes C/D and LNM was significantly higher than that of Dukes A/B and no metastasis (both P<0.05).The level of tissue HER2 in CRC was increased with the decrease of differentiation, but there was not statistically significant (all P> 0.05).4. Correlation in the expressions of serum EGFR(HER2) and tissue EGFR(HER2) of colorectal cancer4.1Correlation in the expressions of serum EGFR and tissue EGFR of colorectal cancer Serum EGFR and tissue EGFR of CRC were high expression for 45 cases and low expression for 10 cases; The expression levels of the serum EGFR was positively correlated with the tissue EGFR in patients of colorectal cancer. (x2=4.603,r=0.240, P=0.032);4.2Correlation in the expressions of serum HER2 and tissue HER2 of colorectal cancer Tissue HER2 and serum HER2 of CRC were high expression for 49 cases and low expression for 7 cases; The expression levels of the serum HER2 was positively correlated with the tissue HER2 in patients of colorectal cancer (x2=5.176,r=0.298, P=0.023).5. Correlation in the expressions of serum EGFR and serum HER2 of colorectal cancer Serum EGFR and serum HER2 of CRC were high expression for 49 cases and low expression for 7 cases; The expression levels of the serum EGFR was positively correlated with the serum HER2 in patients of colorectal cancer. (x2=4.295,r=0.232, P=0.038)6. Correlation in the expressions of tissue EGFR and tissue HER2 of colorectal cancer Tissue EGFR and tissue HER2 of CRC were high expression for 43 cases and low expression for 12 cases; The expression levels of the tissue EGFR was positively correlated with the tissue HER2 in patients with colorectal cancer. (x2=5.142,r=0.250, P=0.023)Conclusion:1.The high levels of serum EGFR and tissue EGFR is significantly associated with the occurrence and development of colorectal cancer, detecting the levels of serum EGFR can reflect the levels of tissue EGFR and which can provide the theory basis for the diagnosis and treatment for the patients with colorectal cancer;2.The high levels of serum HER2 and tissue HER2 is significantly correlated and were closely associated with the occurrence and development of colorectal cancer, detecting the levels of serum HER2 can reflect the levels of tissue HER2, they were was closely correlated to Dukes stage, LMN, thus the levels of serumHER2 can become the maker for the diagnosis and treatment for the patients with colorectal cancer;3. The higher expression of tissue EGFR and HER2 was significantly positively correlated, suggesting that they may play a synergistic role in the development of colorectal cancer4. The higher expression of the serum EGFR and HER2 was significantly positively correlated, which suggest that they may play a synergistic role in the development of colorectal cancer, to combine detection of the leves of serum EGFR and HER2 may provide a new direction for the diagnosis of colorectal cancer.
