Comparative Study On The Debridement Efficacy For Open Fracture Between Two Kinds Of Techniques:Versajet Hydrosurgery And Conventional Surgical Debridement

Objective:To study and evaluate the clinical debridement effects of Versajet Hydrosurgery in the wound bed of GustiloⅢA/ⅢB open fractures treatment.Methods:We selected 40 samples randomly from the patients in the department of orthopedics, The First Affiliated Hospital of Xinjiang Medical University from April 2014 to August 2015. Of the samples we chose, 24 are GustiloⅢA and 16 are GustiloⅢB. All samples were divided into two groups—Hydrosurgery Debridement Group and Conventional Debridement Group. All debridement operations were conducted within 2-6 hours, and the wounds were closed by negative pressure wound therapy(NPWT). Then we compared the time consumed in debridement and wound bed preparation, the numbers of debridement, the level of PCT before and after debridement, and the positive rate of bacterial culture in the two groups. There were no significant difference between these two groups in terms of average age, gender, wound dimensions, fracture classification, and injury times(P>0.05).Results:The time consumed in debridement was significantly less in the Hydrosurgery Debridement Group than in the Conventional Debridement Group(Hydrosurgery Debridement Group 46.00±7.69 minutes, Conventional Debridement Group 50.95±7.23 minutes,P<0.05), the PCT level of Hydrosurgery debridement group after debridement were much lower than the level of traditional debridement group at the same time point(P <0.05).However,There were no significant differences between the two groups in terms of wound bed preparation times(Hydrosurgery Debridement Group 9.89±4.32 days,Conventional Debridement Group 11.38±4.36 days,P>0.05),the numbers of debridement, and the positive rate of bacterial culture(4 cases of Hydrosurgery Debridement Group,3 cases of Conventional Debridement Group,P>0.05). Conclusion:Versajet Hydrosurgery can shorten debridement times, reduce the inflammatory response of body and decreasing the positive rate of bacterial culture. However, with respect to the conventional debridement techniques,the advantage of Versajet Hydrosurgery in shortening the wound bed preparation time, reducing debridement frequency, and decreasing the positive rate of bacterial culture needs further study.
