Objective:To explore the expression and significance of costimulatory molecule PD-L1 in triple negative breast cancer(TNBC).Methods:The first chapter, the immunohistochemical method was used to detect the pathological index including AR and PD-L1 of triple negative breast cancer, and the data were collected and interpreted.The logistic regression was used to analyse the association between clinicopathological parameters. In the second chapter,the thriple negative breast cancer patients were followed up, survival data were collected, and the Kaplan-Meier method was used to estimate the survival rate of the single factor, and the Log-rank test was used to compare the survival curves between groups. COX proportional hazard regression model was used for multivariate analysis of prognostic factors in thriple negative breast cancer patients.Results:The positive expression rate of PD-L1 in TNBC was 28%.Logistic regression single factor analysis showed that the expression of PD-L1 was significantly associated with tumor size, number of lymph node metastasis in TNBC.The expression of PD-L1 was not related to age, menstrual status, histological grade, pathological type, Ki-67 index and so on. Multivariate analysis showed that the tumor size and the number of lymph node metastasis were independent related factors of PD-L1 expression.Single factor analysis showed that PD-L1 expression and tumor size, number of lymph node metastasis, pathological type, chemotherapy or chemotherapy whether complete were the important factors influencing the disease free survival for TNBC. Multivariate COX proportional hazard regression model analysis showed that PD-L1 expression was not an independent risk factor for disease free survival in patients with TNBC. The number of lymph node metastasis was independent risk factor for disease free survival in patients with TNBC. Single factor analysis showed that PD-L1 expression and age, tumor size,the number of lymph node metastasis, staging, whether chemotherapy was performed or whether the chemotherapy was complete, recurrence and metastasis were important factors affecting the overall survival of TNBC patients. Multivariate COX proportional hazard regression model analysis showed that PD-L1 positive expression, age, the number of lymph node metastasis, recurrence and metastasis were the independent risk factors of the overall survival of patients with TNBC.Conclusion:The positive expression rate of PD-L1 in TNBC was high, and the research on PD-L1 targeted therapy in TNBC was feasible. The expression of PD-L1 was related to the size of tumor and the number of lymph node metastasis. PD-L1 may more likely to be positive when the tumor diameter is larger and the number of lymph node metastasis is more. PD-L1 expression may be related to the invasiveness of the tumor. PD-L1 expression in TNBC is an independent risk factor affecting the overall survival of patients, and expected to become a biological indicator to judge the prognosis of TNBC patients.
