Objective:To investigate the patients after cardiac valve replacement self-management status; from social ecology model perspective, analyze the influencing factors of patients self management behavior after cardiac valve replacement. To improve patients self management behavior after cardiac valve replacement, provide a basis to improve the quality of life; To explore the relationship between various influencing factors and self management level.Methods:conducted a questionnaire survey by 220 follow-up patients of cardiac valve replacement in a Shandong tertiary referral hospital between August and December 2015. Tools include questionnaire (including demographic data and disease related information), revised edition of the Morisky medication compliance of table 8 (MMAS-8), chronic disease self management behavior scale, chronic disease management self-efficacy scale, resource of chronic disease questionnaire. Using statistical software SPSS 17.0 for statistical analysis. The statistical methods include descriptive mean, standard deviation, percentage, etc. T test; variance analysis and multiple linear regression analysis to describe the effect factors of cardiac valve replacement patients self management level.Results:1、220 patients with heart valve replacement have an average score of 30.27±11.99 points in self-management behavior, the score of each dimension from high to low in turn to self perception(14.14±6.60), doctors communication (9.94±277), exercise (6.19±4.98).2、According to the Chinese revised version of the Morisky medication adherence scale (MMAS.8) measured in 220 patients with medication compliance score was 7.08±1.21. The high compliance accounted for 48.2%, secondary compliance accounted for 34.1%, low compliance accounted for 17.7%.3、Chronic disease management self-efficacy scale measured the chronic disease self management self-efficacy score was 8.37±1.71, of which the high self-efficacy was 71.8%, self-efficacy was 27.3%, low self-efficacy was 0.9%.4、Using the questionnaire survey of chronic disease prepared by Glasgow to test the 220 survey respondents, the chronic disease score was 53.29±9.93.5、This study shows that main factors affecting the patient self management level are: gender, age,marital status, living conditions, living, culture degree, family per capita income, source of medical costs, occupation, smoking, postoperative course, after the operation time, the types and cardiac function in heart valves, chronic diseases, and self-efficacy.Conclusion:1、Valvular heart disease after valve replacement, patients self management are in the medium low levels,remains to be further improved, the self cognition got the highest score, and exercise got the lowest.2、According to the social ecology model, cardiac valve replacement patient self management influencing factors is multifaceted, including responding to individual factors (gender, nationality, smoking, after the operation time, postoperative the types, self-efficacy in heart valves), closely related (marital status), environment (family income per month), culture (chronic disease resources).3、The valvular heart disease after valve replacement in patients with self management behavior influencing factors can be explained by social ecology model.
