Objectives:The research aims to discuss the relationship of ambulatory pulse pressure and symptomatic intracranial arterial stenosis in ischemic stroke patients. And further assess the correlation between ambulatory pulse pressure and the severity of symptomatic intracranial arterial stenosisMethods:Collect ischemic stroke patients with intracranial arterial stenosis (n=157) as symptomatic intracranial arterial stenosis group. And collect patients with no obvious intracranial arterial stenosis (n=148) as control group. All patients were given ambulatory blood pressure monitoring.24h ambulatory pulse pressure was measured. The correlation between ambulatory pulse pressure and symptomatic intracranial arterial stenosis was evaluated through statistical analysis.Results:1. Relationship of 24h ambulatory pule pressure and symptomatic intracranial arterial stenosis(1) The comparison between the stenosis group and control group in 24h ambulatory pulse pressure.24h ambulatory pulse pressure of stenosis group is (70.25+11.95) mmHg, while control group is (60.10±12.03) mmHg. The value in stenosis group is significantly higher than control group(P<0.01).(2) The comparison between the stenosis group and control group in ambulatory pulse pressure index. Ambulatory pulse pressure index of stenosis group is (0.49±0.03), while control group is (0.44±0.06). The value in stenosis group is significantly higher than control group(P<0.05).(3) The comparison between the stenosis group and control group in 24h ambulatory pulse pressure distribution.24h ambulatory pulse pressure distribution is statistical difference between stenosis group and control group. When 24h APP>60 mmHg, the frequency of symptomatic intracranial arterial stenosis could be significantly increased. And the biggest distribution area of symptomatic intracranial arterial stenosis is≥70 mmHg.(4) The comparison between the stenosis group and control group in ambulatory pulse pressure index distribution.Ambulatory pulse pressure index distribution is statistical difference between stenosis group and control group. When PPI>0.4, the frequency of symptomatic intracranial arterial stenosis could be significantly increased.2. Correlation between 24 ambulatory pulse pressure and severity of symptomatic intracranial arterial stenosis.24h ambulatory pulse pressure of moderate stenosis group (66.73±8.01 mmHg) is significantly higher than asymptomatic intracranial arterial stenosis group (60.10± 12.03 mmHg)(P<0.05). The value in severe stenosis group(74.82+7.80 mmHg) is significantly higher than moderate stenosis group(P<0.05).3. The risk factors of symptomatic intracranial arterial stenosis. The relationship between 24h ambulatory pulse pressure and symptomatic intracranial arterial stenosis was analyzed by using binary Logistic regression analysis. The results showed that the level of 24h ambulatory pulse pressure and ambulatory pulse pressure index were both independent risk factors of symptomatic intracranial arterial stenosis.Conclusion:1.24h ambulatory pulse pressure and ambulatory pulse pressure index were all significantly related to symptomatic intracranial arterial stenosis.24h ambulatory pulse pressure and ambulatory pulse pressure index were the important predictor for symptomatic intracranial arterial stenosis.2.24h ambulatory pulse pressure was closely related to the severity of symptomatic intracranial arterial stenosis.24h ambulatory pulse pressure was the best predictor for the severity of symptomatic intracranial arterial stenosis.3.24h ambulatory pulse pressure and ambulatory pulse pressure index were both closely related to the occurrence of symptomatic intracranial arterial stenosis. They are the risk factors for symptomatic intracranial arterial stenosis.
